Efficacy of the Ketogenic Diet for Intractable Seizure Disorders: Review of 58 Cases

Abstract: The ketogenic diet was developed in the 1920s as a treatment for intractable childhood seizures when few antiepileptic drugs (AEDs) were available. There are still children whose seizures are refractory even to modern therapy, but use of the ketogenic diet appears to be waning. At Johns Hopkins, we continue to believe that the diet is very effective and well accepted by patients and families. To reevaluate our opinion of the efficacy and acceptability of this form of therapy in patients cared for in the 1980s … Show more

“…[1][2][3][5][6][7]21,22,27,28 Historically, it has been established that brain can utilize ketone bodies under ketotic conditions. 1,4,10,[13][14][15][16]39 However, corresponding changes in CMR glc during 'metabolic adaptation' to ketosis has not been clearly described. Using PET-FDG imaging, the focus of this study was to determine if CMR glc decreases with increasing ketosis in adultanesthetized diet-induced ketotic rats.…”

“…The ketogenic diet (high-fat, very low-carbohydrate) to induce chronic ketosis has been successfully used in the clinical setting as a therapy for intractable seizures for nearly a century. 9,[12][13][14][15] However, the 'mechanistic link' between the anticonvulsant effects and ketosis continues to be investigated and remains to be elucidated. 16 Ketone bodies as neuroprotective agents appear to be related to the change in the regulation of the cell's stress responses, 17 as well as changes in oxidative (glucose) metabolism.…”

“…Although one study reported a better response in generalized than focal epilepsies, 16 others failed to identify any significant difference in response according to the type of epilepsy. 17,18 We therefore decided to exclude this factor and to concentrate on a single type of epilepsy, selecting patients with focal epilepsy because they represent one major group of pharmacoresistant epilepsy.…”

“…3 However, studies that addressed the effects of the ketogenic diet according to the type of seizures or epilepsy failed to identify any significant difference. 17,18 Animal studies have concentrated more on models for generalized convulsions 19,20 than on focal seizures, 21 although in both instances ketosis proved to be efficient. These models failed to generate any hypothesis regarding possible mechanisms of antiepileptic and anticonvulsive properties, despite the increase in neurogenesis after kainic-acid-induced seizures in mice.…”

“…A redução de DAE ocorreu em 64% dos pacientes sendo observado maior alerta e melhora do comportamento em 36% dos casos. 17 Uma revisão sistemática de onze estudos sobre a eficácia da DC realizado por Lefevre et al, 18 mostrou uma melhora completa das crises em 16% (intervalo de confiança de 95%) e uma melhora superior a 90% de controle de crise em 32% (intervalo de confiança de 95%).…”
