Effects of the constituents from Ginkgo biloba extract (GBE) on the action potentials and the ionic currents in guinea pig ventricular cardiomyocytes were investigated using whole-cell and current-clamp techniques. The constituents, ginkgolides A, B, C and quercetin, had depressant effects at 0.1–3μM on the action potential configuration. Ginkgolide A (1–3 μM) prolonged the action potential (action potential duration: APD) at 75% and 90% repolarizations (APD75 and APD90). However, ginkgolides B and C at low concentrations prolonged APD, but at higher concentrations (>1 μM) shortened APD. Quercetin at 3 μM prolonged the APD, but not at the lower concentrations. These constituents also inhibited the Vmax. The resting potential was unaffected. In voltage-clamp experiments, ginkgolides A and B (0.1–3 μM) markedly and concentration-dependently increased the Ca2+ current (ICa) and the delayed rectifier K+ current (IK), and decreased the inwardly rectifying K+ current (IK1). On the other hand, ginkgolide C failed to affect the ICa but increased the IK by 14.0 ± 2.3% (n = 6, P < 0.05) at 1 μM. Quercetin inhibited ICa, and enhanced IK but decreased IK1. These responses to the constituents were almost reversible (80–90% of control) after a 10- to 20-min washout. These results indicate that even at acute administrations, these constituents produce the effective actions on the APD and the underlying ionic currents in cardiomyocytes. Each constituent does not exhibit a uniform response, although GBE acts as a net.

Forty patients diagnosed as suffering from senile dementia of the Alzheimer type received either 80mg Ginkgo biloba special extract (GBE)* or matching placebo t.i.d. for three months in a randomized, double blind study of the efficacy and tolerance of GBE. The patients were assessed using a test battery at baseline and at 1, 2 and 3 months. The test battery included the SKT (a brief test of cognitive function, memory and attention), the Sandoz Clinical Assessment Geriatric Scale, choice reaction time, saccadic eye movements and EEG. Memory and attention, as measured by the SKT, improved significantly in the active treatment group after one month, as did psychopathology, psychomotor performance, functional dynamics and neurophysiology as measured by the above tests. The drug was well tolerated and no adverse drug reactions were recorded during the trial.

This overview is focused on the stress-alleviating and vigilance-enhancing properties of Ginkgo biloba extract (EGb 761) that have been observed in animal models and in humans and that appear useful in explaining the therapeutic effects of the extract on non-cognitive brain functions that may be reflective of mood changes. With regard to its stress-alleviating activity, in rodents EGb 761 facilitates behavioral adaptation, decreases circulating levels of corticosterone, catecholamines, ACTH, and CRH, suppresses the expression of the mitochondrial peripheral benzodiazepine receptor of the adrenal cortex, increases the anxiolytic activity of diazepam, increases arousal by an action that may involve GABA A receptor antagonism, induces anxiolytic-like activity, prevents stress-induced desensitization of hippocampal 5-HT 1A receptors, suppresses the down-regulation of hippocampal glucocorticoid receptors, and facilitates decision-making. In humans, EGb 761 has vigilance-enhancing effects that support its clinical use in treating disorders involving stress-induced brain dysfunction and those non-cognitive symptoms of dementia currently classified as ''behavioral and psychological symptoms of dementia'' (BPSD). Collectively, these findings indicate that EGb 761 is useful for treating symptoms that are now classified as components of BPSD. As drug combinations consisting of EGb 761 and a non-selective cholinesterase inhibitor, an antidepressant drug, or a neuroleptic could possibly be even more useful in treating BPSD, evaluation of such therapy is recommended.
