The term ?deciduosis? is used to describe the severe pregnancy-associated occurrence of ectopic decidua with a usually asymptomatic course. We report on two cases of massive maternal intra-abdominal bleeding due to such symptomatic changes. The complications arose at different time points for the two cases: prepartum (26th week of pregnancy) or, respectively, ? reported here for the first time ? seven days postpartum. As well as differential diagnostic aspects we describe the management of the disease and its possible effects on subsequent pregnancies.

EinleitungBei der Deziduose handelt es sich um eine schwangerschaftsassoziierte großzellige Stromareaktion, die in unterschiedlichen Geweben auftreten kann, welche im physiologischen Zyklusgeschehen hierzu nicht in der Lage ist [1]. Das früheste Auftreten einer solchen Reaktion in der Schwangerschaft wurde in der 9. SSW beobachtet und betraf das Ovar [1]. Gegen Ende der Schwangerschaft kommt es zur Regression, die sich über einen Zeitraum von mehreren Monaten post partum [4] erstrecken kann. Nach den in der Literatur beschriebenen Fällen [5] kommt ektope Dezidua in nahezu allen Schwangerschaften vor und wird meist als reiner Zufallsbefund erhoben. Seltener wurde eine ektope Deziduose im Zusammenhang mit einer trophoblastären Neoplasie oder mit hormonbildenden Tumoren von Ovarien und Nebennieren sowie nach Gestagenbehandlungen und Radiatio Zusammenfassung Die Deziduose ist eine schwangerschaftsassoziierte Stromareaktion, die alle Gewebearten betreffen kann. In seltenen Fällen und bei entsprechendem Ausmaß kann sie zu massiven Blutungskomplikationen in der Schwangerschaft führen. Wir stellen den Fall einer Primgraviden vor, die in der 26. SSW primär unter dem Verdacht eines nekrotisierenden Seitenwandmyoms aufgenommen und 14 Tage später unter dem Bild einer intraabdominalen Massenblutung laparotomiert werden musste. Die histologische Differenzialdiagnostik wird ausführlich diskutiert. We present the case of a primigravida who initially was admitted in the 26th week of gestation with the diagnosis of necrotising lateral myoma. A fortnight later she required emergency laparotomy due to extensive intraabdominal bleeding. We intensively describe and discuss the histological differential diagnostic procedures.

Currently, surgical treatment aimed to exclude the malignant ovarian tumors is performed in almost 90% of patients with decidualized endometrial cysts (DEC). However, unnecessary surgical interventions increase the risk to maternal and fetal health. The study was aimed to perform a differential diagnosis of DEC in pregnant women in order to define the rational treatment. A total of 82 female patients were included in the study: 63 had endometrial cysts (EC), 16 had DEC, 3 had rare forms of endometriosis, and 10 had ovarian serous papillary borderline tumors. When performing the diagnostic ultrasound, our proposed model was used. The ultrasound imaging data obtained were juxtaposed with the concentration of the protein tumor markers (СА-125), the risk of malignancy index (RMI) was calculated, and the morphological assessment of the masses was performed. The ultrasound imaging parameters, being the most valuable for differential diagnosis of EC, DEC, and serous borderline tumors, were as follows: the altered mass wall thickness, the existence and shape of papillary masses, avascular echogenic inclusions with blurry contour, blood circulation and arrangement of blood vessels, ascites. The frequency analysis revealed the differences between groups based on the ultrasound imaging data (in 60–100% of observations). Histological examination revealed the differences between groups in 100% of observations. Our findings have made it impossible to prolong pregnancy in patients with DEC without performing surgery. The results of treatment provided to patients with DEC during pregnancy were worse compared to those in patients with no prominent decidualization in ovarian EC. Today, the diagnosis of DEC and the treatment of patients during pregnancy remain unsophisticated. Further clinical observation and the search for more reliable methods of the diagnosis and rational treatment of pregnant women with DEC are required.
