Electronic health record adoption and health information exchange among hospitals in New York State

Abstract: EHR adoption rates and participation in HIE are higher among New York hospitals than hospitals nationwide, suggesting that state initiatives funding community EHR implementation may influence these efforts by hospitals. However, overall rates of adoption and preparedness to meet meaningful use remain low. Direct support for hospitals, such as that provided through the national EHR Incentive Program, will likely be critical for rates of EHR adoption and HIE to significantly rise, even in advanced states.

“…While the goal of creating integrated EHRs is within our reach, its success will depend chiefly on the creation and especially adoption of standards by all international parties [191]. HIE which will enable data integration, semantic interoperability, and CDS across multiple EHRs has been a desired but largely unattained aim of clinical informatics, especially in commercial EHR systems [192,193]. The opportunity for enabling such scalable CDS will require vendor-supported, web-based CDS development platforms along with vendor-supported application programming interfaces (APIs) allowing the use of innovative, pluggable, interchangeable, and API-based CDS applications.…”

“…There have, for example, been initiatives, often driven by government regulations or financial stimulations, in the USA [1], the United Kingdom [2] and Denmark [3]. EHR implementation initiatives tend to be driven by the promise of enhanced integration and availability of patient data [4], by the need to improve efficiency and cost-effectiveness [5], by a changing doctor-patient relationship toward one where care is shared by a team of health care professionals [5], and/or by the need to deal with a more complex and rapidly changing environment [6].…”

“…EHR systems can be used in individual organizations, as interoperating systems in affiliated health care units, on a regional level, or nationwide [1, 2]. Health care units that use EHRs include hospitals, pharmacies, general practitioner surgeries, and other health care providers [7].…”

“…In recent years, an increasing number of hospitals around the world have implemented electronic health record (EHR) systems [1-3]. According to 2015 American Hospital Association survey data, 83.8% of all US non-federal acute care hospitals have adopted at least a basic EHR, representing an almost nine-fold increase since 2008 (9.4%) [4].…”
