Endometrial stromal sarcoma of the uterus presenting as pulmonary metastasis

Abstract: Endometrial stromal sarcoma (ESS) is a rare uterine sarcoma. Compared with other uterine malignancies, it occurs at an earlier age (42-58 years) and about 10-25% of the patients are premenopausal. The tumours have an indolent growth, with a tendency for late recurrence. Metastases are rarely detected before the diagnosis of the primary lesion. We report a case of ESS with pulmonary metastasis as a prodromal manifestation.

“…The Magnetic Resonance Cholangiopanc-reatography (MRCP) and Computed Tomography (CT) on the gallbladder sarcomatoid carcinoma ( Supplementary Figure S1 ) demonstrated interruption of extrahepatic bile duct structure and gallbladder wall thickening, suggesting that the patient might have malignant lesions. The immunohistochemistry staining of the tumor specimens showed pan-cytokeratin (pan-CK) (+), Ki67 (+), Heppar-1 (−), CAM5.2 (+), Vimetin (+), CK19 (−), and SMA (+) ( Supplementary Figure S2 ), which are associated with the clinical characteristics of sarcoma patients [ 35 38 ].…”

“…Pulmonary metastasis is a common occurrence in cases of metastatic carcinoma. In relation to gynecologic tumors, the lungs are reported to be the most frequent site of metastasis from lowgrade endometrial stromal sarcoma (36)(37)(38), although this is uncommon (35). However, recent reports indicate that lung metastasis is the most common distant organ metastasis in endometrial tumors (39)(40)(41), with carcinosarcoma having a significantly higher rate of lung metastasis compared to other histological types, and undifferentiated tumors had the highest rate of lung metastasis when considering tumor grade (29).…”
