Endoplasmic Reticulum Stress in Chemotherapy-Induced Peripheral Neuropathy: Emerging Role of Phytochemicals

Abstract: Chemotherapy-induced peripheral neuropathy (CIPN) is a significant dose-limiting long-term sequela in cancer patients undergoing treatment, often leading to discontinuation of treatment. No established therapy exists to prevent and/or ameliorate CIPN. Reactive oxygen species (ROS) and mitochondrial dysregulation have been proposed to underlie the pathobiology of CIPN. However, interventions to prevent and treat CIPN are largely ineffective. Additional factors and mechanism-based targets need to be identified t… Show more

“…ER stress has an important role in the induction of pain, particularly in neuropathic pain conditions [11,12,75]. Neuropathic pain, often resulting from nerve damage or a malfunctioning nervous system, is notoriously difficult to treat [12,75,76]. The role of ER stress in pain is not limited to neuropathic pain but extends to various medical conditions, including diabetes-induced neuropathic pain, inflammatory pain, and pain associated with diseases like osteoarthritis and viral infection.…”

“…Neuroprotective drugs including duloxetine, amifostine, B vitamins, antioxidants such as vitamin E, alpha‐lipoic acid, glutathione and glutamine, and minocycline have certain effects on the prevention and treatment of CIPN. However, evidence‐based medicine is insufficient, and it can be used clinically as appropriate 102–104 . The American Society of Clinical Oncology (ASCO) guidelines states that duloxetine as a serotonin–norepinephrine reuptake inhibitor is the only recommended treatment 6 .…”

“…Long term ER stress activates c-Jun N-terminal kinase (JNK) and p38 MAPK, thereby promoting apoptosis and necrosis (11). In addition, ER stress engages mitochondria in the process of apoptosis through Ca 2+ release (12). Therefore, to achieve effective results, interventions that target ER stress based on apoptosis or oxidative stress need to be developed.…”
