Background: The use of the exoscope has been increasing in the field of neurosurgery, as it can set the visual axis freely, enabling the surgeon to operate in a comfortable posture. Although endoscope-assisted surgery for compensation of insufficient surgical field is useful under the microscope, we report that using an endoscope in exoscopic surgery is safer and more useful. Methods: The exoscope used was ORBEYE. All surgical procedures were performed exoscopically from the beginning of the surgery. When endoscopic observation was required during the operation, the endoscope was inserted under observation by an exoscope. The exoscopic screen was 4K-3D and endoscopic screen was 4K-2D, the operation was performed while observing both screens at the same time. The endoscope was held manually or by a mechanical holder. Results: Twenty-two cases, including 14 requiring microvascular decompression (MVD) and eight requiring tumor removal, were performed by endoscopic-assisted exoscopic surgery. The endoscope could be inserted safely because its relationship with the surrounding structure could be observed under the exoscope, and the operator could observe both screens without moving the head. Fourteen of 22 patients required additional endoscopic treatment. Satisfactory two-handed operation was performed in 13 cases. Symptomatology disappeared in all cases of MVD, and sufficient tumor resection was achieved. Conclusion: Exoscopic surgery provides excellent surgical view that is not inferior to conventional microsurgery. As a large space can be secured between the scope and the surgical field, it is safer and easier to manipulate the endoscope under the exoscope.

Гемифациальный спазм (ГФС) – заболевание, проявляющееся безболезненными пароксизмальными непроизвольными односторонними тоническими или клоническими сокращениями лицевой мускулатуры, иннервируемой ипсилатеральным лицевым нервом. Частота встречаемости ГФС составляет примерно 11 на 100 000 населения, при этом заболевание манифестируется в среднем в районе третьего десятка жизни пациента. Гемифациальный спазм подразделяется на первичный, когда наличие спазмов обусловлено нейроваскулярным конфликтом в области лицевого нерва, и вторичный, когда наличие фасцикуляций лицевой мускулатуры обусловлено другими причинами. Первичный гемифациальный спазм чаще всего возникает из-за сдавления лицевого нерва артерией (передней нижней мозжечковой, задней нижней мозжечковой, позвоночной), реже венозными сосудами в области выхода нерва из ствола головного мозга. Учитывая распространенность гемифациального спазма, поражение преимущественно пациентов молодого возраста, инвалидизирующий характер заболевания, изучение этиологии, патогенеза, анатомических осбенностей первичного ГФС является актуальной проблемой современной нейрохирургии. Hemifacial spasm (HFS) is a disease manifested by painless paroxysmal involuntary unilateral tonic or clonic contractions of the facial muscles innervated by the ipsilateral facial nerve. The incidence of HFS is approximately 11 per 100,000 population, and the disease manifests itself on average around the third decade of the patient’s life. Hemifacial spasm is divided into primary – when the presence of spasms is due to a neurovascular conflict in the area of the facial nerve and secondary – when the presence of fasciculations of the facial muscles is due to other reasons. Primary hemifacial spasm most often occurs due to compression of the facial nerve by an artery (anterior inferior cerebellar, posterior inferior cerebellar, vertebral artery), less often – by venous vessels in the area where the nerve exits the brain stem. Considering the prevalence of hemifacial spasm, the affection of predominantly young patients, the disabling nature of the disease, the study of the etiology, pathogenesis, and anatomical features of primary HFS is an urgent problem of modern neurosurgery.

Co giật nửa mặt (hemifacial spasm) biểu biện bởi các chuyển động giật rung hay co cứng tiến triển, không tự chủ, không thành cơn của các cơ chịu sự chi phối của thần kinh VII ở một nửa bên mặt. Nội soi hỗ trợ vi phẫu thuật giải ép mạch máu thần kinh trong điều trị bệnh lí co giật nửa mặt đang là xu hướng mới ở Việt Nam. Nghiên cứu mô tả tiến cứu 11 bệnh nhân co giật nửa mặt được vi phẫu thuật giải ép có nội soi hỗ trợ tại Bệnh viện Việt Đức từ 1/2021 đến 12/2021. Nguyên nhân hay gặp nhất gây xung đột mạch máu thần kinh là động mạch tiểu não trước dưới chiếm 72,7%. Triệu chứng lâm sàng sau phẫu thuật: 45,45% hết triệu chứng các trường hợp, 54,55% giảm triệu chứng. Trong 6 trường hợp được đánh giá kết quả phẫu thuật bằng thang điểm HFS, có sự giảm rõ rệt điểm HFS: lâm sàng từ 12 - 16 điểm trước mổ; giảm còn 0 - 4 điểm sau mổ); có sự giảm rõ rệt điểm HFS - Chất lượng cuộc sống 40 - 72 điểm trước mổ; giảm còn 0 - 16 điểm sau mổ). Vi phẫu thuật giải ép mạch máu thần kinh có áp dụng nội soi hỗ trợ là biện pháp hiệu quả trong điều trị co giật mặt, ít biến chứng. Bộ câu hỏi HFS - Lâm sàng và Chất lượng cuộc sống có thể áp dụng trong theo dõi và đánh giá kết quả sau phẫu thuật.
