Endoscope‐assisted resection of thyroglossal duct cysts via a submaxillary vestibular approach

Abstract: Endoscope-assisted excision of TDCs using a submaxillary vestibular approach is feasible and can be applied effectively and safely with good cosmetic results. Therefore, this method should be considered a viable alternative approach for these patients.

“…[7][8][9] A transoral approach with a single vestibular incision without insufflation has also been reported. 10 This is the first published report of a novel three-port transoral vestibular Sistrunk operation for a TGDC. Our technique is adapted from that of a transoral endoscopic thyroidectomy vestibular approach (TOETVA), which has been well described and also used for parathyroidectomy and central neck dissections.…”

“…The 24 hours of pressure dressing minimizes the risk of hematoma and seroma. Additionally, a small drain could be placed through the cervical skin flap or transorally 10 to prevent any fluid accumulation. A drain was not required in this case since hemostasis was easily achieved and he had no risk factors for bleeding.…”

“…Kim et al (7) and Lee et al ( 8) selected the retroauricular approach, successfully removed TGDC with robot assistance, and the incision can be hidden, but the costs and hardware constraints obstruct wider accessibility. Han et al (9) and Banuchi et al (10) completed the operation by employing the transoral vestibular approach. The former adopts a single incision without insufflation, while the latter adopts the same three incisions with CO 2 insufflation as the vestibular approach for thyroid.…”

“…Both operations were completed successfully without complications. However, the single incision method of Han et al (9) does not avoid the problem of narrow operation space. Asians have a relatively flat jaw, which may impede the use of this technology among other races.…”
