Endoscopic Resection of Sinonasal Cancers With and Without Craniotomy

Abstract: To the best of our knowledge, this is the largest US series to date of patients with malignant tumors of the sinonasal tract treated with endoscopic resection. Our results suggest that, in well-selected patients and with appropriate use of adjuvant therapy, endoscopic resection of sinonasal cancer results in acceptable oncologic outcomes.

“…We performed orbital exenteration (7) and CFR (4) when there was significant periorbit and dural invasion, respectively. As opposed to other series [7,10], we do not support low threshold for performing CFR because we argue that the high rate of surgical complication and morbidity overpass the survival rates improvements.…”

“…Interestingly, while positive margin status has been shown to be adverse in sinonasal cancer resection, surgery with widely negative margins isn't clearly better than more conservative ones, as long as clinically negative margins are achieved. Further, although no large, or randomized, studies have been performed for comparison, piecemeal resection via endoscopic approaches appears to be as effective as en bloc resection [3,29]. Precision radiotherapy, such as intensity modulated radiation therapy and gamma knife have improved treatment and lessened morbidity, particularly for those patients with tumors adjacent to the orbits, skull base, and brain.…”

“…Средний период наблюдения составил 37 мес. Локального контроля удалось достичь у 85% паци-ентов, при этом уровень 5-и 10-летней выживае-мости в зависимости от болезни составил 87 и 80% соответственно [20]. Согласно Европейскому доку-Рис.…”
