Endothelium-dependent and cyclooxygenase-dependent cutaneous vasodilatation is blunted in young men with hypertensive parents

Abstract: Reactive hyperaemia and ACh-evoked dilatation in cutaneous circulation are blunted in young, normotensive OH relative to ON men irrespective of white European, or South Asian ethnicity and are attributable to impaired contribution of COX vasodilator products in OH. These features may provide early markers of endothelial dysfunction that contribute to hypertensive risk in OH men.

“…These issues are discussed in more detail below. The present finding that cutaneous dilatation during reactive hyperaemia and in response to ACh was blunted in South Asian ON and South Asian OH women relative to white European ON women, is comparable to our observations in young men who were South Asian or white European and of ON or OH status [12]. We recognize that structural limitation on maximal dilatation of arterioles and capillary rarefaction occurs in essential hypertension [26].…”

“…Moreover, in our recent study, young normotensive South Asian men showed blunted dilator responses to acetylcholine (ACh) and reactive hyperaemia in cutaneous microcirculation compared with young normotensive white European men. In both ethnicities, these endothelium-dependent dilator responses were blunted in OH relative to ON men and the disparity was attributable in part to reduced contribution of cyclooxygenase (COX) products, most likely vasodilator prostaglandins [12]. Taken together, this evidence suggests that early markers of endothelial dysfunction, a correlate of CVD, are already present in young South Asian men, especially in those at high familial risk of hypertension.…”

“…Although more prevalent in advanced ages, some pathophysiological abnormalities such as altered autonomic control, 3,4 increased cardiac sympathetic neurotransmission, 5 endothelial dysfunction, 6 and vascular endoplasmic reticulum stress 7 were already observed in young-to middle-aged hypertensive individuals. Experimental evidence suggests that angiotensin II (Ang II) and angiotensin- (1)(2)(3)(4)(5)(6)(7) [Ang-(1-7)], the main active hormones of the renin-angio-tensin system (RAS), are essential players in hypertensioninduced abnormalities. 8,9 Among hypertension-induced end-organ lesions, hypertrophic cardiac remodeling is a well-known characteristic.…”

“…В целом, анализ литературы выявил достаточное количество работ [11,14,16], демонстрирующих первичную роль ЭД в развитии АГ у лиц с отягощенной наследственностью по ССЗ, в т. ч. у подростков и молодых людей [17,18].…”
