“…On the contrary, the endotracheal administration of a combination of prophylactic polymyxin B plus aminosidin in 22 tracheotomized patients was associated with decreased incidence of broncholpumonary infections caused by gentamicin resistant strains compared with the endotracheal administration of gentamicin. The investigators concluded that the combination of local administration of polymyxin B with aminosidin to the respiratory tract represents an alternative to gentamicin for the prevention of pulmonary infections [26]. In addition, a double-blind, placebo-controlled, nonrandomized clinical trial assessed the effect of prophylactic polymyxin B aerosol administered to the posterior pharynx and tracheal tube on the incidence of pneumonia in 744 seriously ill patients.…”

“…Estudos admitem, como maior efeito adverso para a administração de antibióticos inalatórios a seleção de cepas antibiótico-resistentes, porém estes trabalhos correlacionam esse fato com o uso profilático, 11 sugerindo ser minimizado o problema com a alternância de drogas 12,13 . Thys, Aoun & Klastersky, em 1994, descreveram esta resistên-cia como rara e transitória, ocorrendo apenas após muitos meses de uso contínuo.…”

“…However, for six strains of bacteria isolated from the gentamicin-treated group, the MICs were higher than 6 ,ug/ml; this was true for only two strains from the placebotreated group. In a subsequent study by the same group, gentamicin was compared with a combination of aminosidin (an aminoglycoside) and polymyxin B given as an endotracheal aerosol (32). The number of infections in the two groups was similar, but all strains causing infections in the gentamicin-treated group were resistant to gentamicin, whereas those causing infections in the other group did not show such a high rate of resistance to aminosidin or polymyxin B.…”

“…ANTIMICROBLIL RESISTANCE The most obvious risk of the widespread application of antimicrobial prophylaxis, as was shown in the early studies noted above (16,(32)(33)(34), is the development of antimicrobial resistance. Although the development of resistance was searched for in all but one of the studies summarized in Tables 1 and 2, an increase was noted in only 5 of the 17 studies (4,(33)(34)(35).…”
