Endovascular Repair of a Blunt Abdominal Aortic Injury

Abstract: Blunt abdominal aortic injury is an uncommon traumatic finding. In the past, treatment options have traditionally consisted of open operative repair; however, the development of endovascular surgery has created new interventional possibilities. This case is presented to demonstrate the applications of endovascular abdominal aortic repair for a blunt traumatic injury.

“…Endovascular treatment has proven to be a successful advancement in the management of vascular trauma. Numerous case reports established the efficiency of endovascular management in blunt aortic injury since the first report of successful endovascular repair of a traumatic abdominal aortic dissection by Marty-Ane et al in 1996 [20][21][22][23][24][25][26].…”

“…Injuries of the abdominal aorta secondary to blunt trauma are rare; they are reported to occur in less than 1% of blunt traumas. 2,11 The most common inciting trauma is motor vehicle collisions. Seat belts in deceleration collisions can contribute to TAAI (“seat belt aorta”).…”

“…Endovascular repair of TAAI is safe and feasible if the patient does not require urgent abdominal exploration. 2 The following case outlines a patient who suffered blunt trauma resulting in a TAWH and an abdominal aortic pseudoaneurysm.…”

“…Seat belts have since been upgraded to a three-point restraint, which results in fewer associated spinal injuries; and thus a new "seat belt lethal triad" has been described, which is made up of TAWH, hollow viscus injury, and abdominal vascular injury (4,41). Although injuries to the abdominal aorta are uncommon, occurring in less than 1% of blunt trauma cases, the mechanism for injury is the same as that causing TAWH; and thus a high index of suspicion should be maintained (44,45).…”
