Enteric Autoantibodies and Gut Motility Disorders

Abstract: There is increasing evidence that a subset of gastrointestinal motility disorders is associated with the presence of circulating antibodies. These antibodies are directed against various molecular targets, the best known being anti-neuronal nuclear antibody (ANNA-1, aka anti-Hu) associated with paraneoplastic motility disorders. There is also evidence that the presence of distinct auto-antibody profiles is associated with non-paraneoplastic motility disorders. This review will focus on the types of antibodies … Show more

“…The UNC-OSU group evaluated the prevalence of anti-ENS autoimmune antibodies and the antigens recognized by the antibodies in patients diagnosed with idiopathic IBS. The approach was based on one used productively for evaluation of anti-enteric neuronal antibodies in patients with gastrointestinal disorders associated with a paraneoplastic syndrome [37][38][39][40][41][42].…”

“…Finally, if the clinical picture is highly suggestive of an occult malignancy despite a negative initial work-up, bronchoscopy or mediastinoscopy may be offered to the patient. 1 In the case of our patient, his initial chest CT was negative except for a few enlarged precarinal lymph nodes. It was decided that a lymph node biopsy would be offered to the patient if the PET scan failed to show a source.…”

“…These autoantibodies are thought to target tumor antigens that show cross-reactivity with myenteric neurons. 1 Small cell lung carcinoma and thymoma are the most common neoplasms associated with paraneoplastic intestinal dysmotility due to ANNA-1 or anti-Hu antibodies. 4,5,15 However, associaton with lymphomas and tumors of the breast, prostate and ovary have also been reported.…”

“…Esta se manifiesta como pseudoacalasia (102), gastroparesia y pseudoobstrucción intestinal (38,103,104). La gastroparesia es la más común (105), a diferencia de la pseudoacalasia paraneoplásica, que es muy rara (1 persona en cada 750 000) (106).…”
