Entrustable Professional Activity 10: Case Simulation and Assessment—STEMI With Cardiac Arrest

Abstract: Introduction: Entrustable professional activities (EPAs) are units of professional practice defined as tasks or responsibilities that trainees are entrusted to perform unsupervised. AAMC Core EPA 10 is defined as the ability to "recognize a patient who requires emergent care and initiate evaluation and management." We designed a simulation scenario to elicit EPA 10-related behaviors for learner assessment to guide entrustment decisions. Methods: This case presents a 61-year-old male with a complaint of feeling… Show more

“…Kman et al . proposed an EPA test for clerkship through simulation of a diagnosis of a myocardial infarction with ST-segment elevation with cardiac arrest [ 14 ]. Approximately 86% of students reached the judgment of entrustment at the end of the test.…”

“…There is a published case of STEMI in MededPORTAL [7] that can complement this case. Our simulation is unique in its particular emphasis on collaborative teamwork, communication skills between medical and nonmedical staff, and administrative process, so that this team can achieve the appropriate time to definitive treatment with door-to-balloon time ≤90 minutes.…”

“…As UME continues to adopt competency-based medical education, EPAs offer a complementary assessment system based on holistic, observable, and behavioral determinants of performance [ 8 , 9 ]. Current approaches to EPA assessment leverage existing medical school clerkships, simulation centers, or capstone programs [ 10 , 11 ]. Although convenient, these traditional methods of student assessment often fail to collect adequate data for competency decisions across all 13 EPAs.…”
