EoE disease monitoring

Abstract: The burden of endoscopy in eosinophilic esophagitis (EoE) is great, with many patients undergoing multiple endoscopies to verify treatment success. The gold standard for monitoring disease activity EoE is histologic evaluation and eosinophil enumeration. Although symptoms have been less reliable to track disease activity, multiple noninvasive methodologies are being investigated to sample the esophageal tissue. A better understanding of disease phenotype, genotype, and transcriptome may lead to more personaliz… Show more

“…Since the pathophysiology of EoE, BA, and AD is very similar, it is likely that unspecific biomarkers could be found when using samples such as blood the most common tissue used when looking for non-invasive biomarkers and therefore, will be the focus of this review. It should be noted that overall biomarkers for EoE [ 43 ], BA [ 44 ], and AD [ 45 ] have been reviewed recently. As counts of circulating eosinophil and serum IgE levels have been widely employed in EoE, BA, and AD, with inconclusive results, they will not be reviewed here.…”

“…With the rising prevalence and incidence of EoE, and as diagnosis requires the identification of esophageal eosinophilia, an emerging challenge in management is disease monitoring. 178,179 As symptoms of EoE are not consistently reliable in regard to correlation with disease activity, disease monitoring in EoE commonly requires repeated endoscopy with biopsies. 180 The development of minimally invasive monitoring tests for disease activity in EoE is an area of active investigation.…”
