Aims: To evaluate the clinical stage at diagnosis of prostate cancer. Materials and Methods: This study was retrospective and descriptive from January 2008 to June 2013. Patients who were diagnosed with prostate cancer wereinvolved and confirmed by clinical examination anatomopathological screening. They also realized a total serum PSA, a bladder-prostatic and abdominal ultrasound. Results: Average age was seventy years, ranging from fifty years to seventy years. The general status of our patients was satisfactory in 78% of cases. Main symptoms were dysuria and frequencyin 96% and 86%, respectively. Family past history of prostate cancer were found in 24% of cases. Twenty-three patients (43%) had a total serum PSA ≥ 100 ng/ml. Adenocarcinoma was the main histological type. Grades 1 and 4 were the most frequent. Thirty-three patients (66%) had a Gleasonscore betwen [2]-[6]. Forty-four patients (88%) had locally advanced and metastatic desease. The D'Amico score was identified in six patients but 3 and 2 patients were low risk and intermediate riskrespectively. Finally, 1 patient was high risk. Conclusion: Prostate cancer remains elderly man cancer. It was discovered in locally advanced and metastatic desease in this series. This diagnosis was late in the majority of cases.

Le cancer de la prostate pose à l'heure actuelle un problème de santé publique avec une fréquence variable selon les pays. Objectif : Notre étude avait pour but de décrire les aspects épidémiologiques, cliniques et anatomopathologiques des cancers de la prostate en Côte-d'Ivoire. Matériel et méthodes : Il s'agissait d'une étude rétrospective et descriptive portant sur les cancers de la prostate recensés à partir des registres des laboratoires d'anatomie pathologique des centres hospitaliers et universitaires (CHU) d'Abidjan. La période d'étude était de 24 ans (1984 à 2007). Les paramètres étudiés étaient : la provenance des prélèvements, la fréquence, l'âge, la clinique, la biologie (le taux de PSA, Prostatic Specific Antigen) et les aspects anatomopathologiques et pronostiques. Résultats : Les cancers de la prostate représentaient 7,72 % (n = 916) des cancers, soit 26,43 % des cancers de l'appareil urogénital. Ils occupaient le deuxième rang des cancers de l'homme après les cancers cutanés et le troisième rang des cancers tous sexes confondus après les cancers du col utérin et ceux de la peau. L'âge moyen des patients était de 67,81 ans (extrêmes de 39 et 93 ans). La tranche d'âge de 65 à 74 ans était la plus touchée (39,34 %). Les sujets âgés (≥ 65 ans) représentaient 61,24 % des cas (n = 561). Le taux moyen de PSA était de 121,94 μg/ml (extrêmes de 3,25 à 5 107 μg/ml). L'adénocarcinome était le type histologique prépondérant (93,67 %) dont 6,08 % étaient diagnostiqués sur pièce d'adénomectomie. Au plan pronostique, les scores de Gleason supérieurs ou égaux à 7 représentaient 57,25 % des cas (n = 296). Conclusion : Les cancers de la prostate sont fréquents et de mauvais pronostic dans notre pays. La mise en place d'une politique de dépistage individuelle constituerait un avantage indéniable dans l'amélioration du pronostic. Patients et méthodes : Il s'agissait d'une étude rétrospective bicentrique de cinq ans portant sur 91 prostatectomies radicales réalisées dans le centre médicochirurgical de Chaumontle-Bois en France et la clinique Notre-Dame-de-la-Paix au Burkina Faso. Résultats : En cinq ans, 91 patients ont bénéficié d'une prostatectomie radicale à visée curative dont six à la clinique Notre-Dame-de-la-Paix au Burkina Faso et 85 dans le centre médicochirurgical le Bois en France. Leur âge moyen était de 66,52 ans. Cliniquement, les patients étaient classés stade A (2,2 %) ou B (98,8 %) de Whitemore-Jewett. Le taux moyen de prostatic specific antigen (PSA) était de 9,25 ng/ml. L'échographie endorectale, la tomodensitométrie abdominopelvienne, la scintigraphie osseuse, l'imagerie par résonance magnétique et l'histologie des pièces biopsiques avaient permis de classer tous les patients en T1a à T2b N0M0. Il s'agissait d'adénocarcinomes, et le score moyen de Gleason était de 6. Le traitement chirurgical était une prostatectomie radicale. Après l'étude anatomopathologique des pièces opératoires, 2,2 % étaient classées pT1, 76,92 % pT2, 21,92 % pT3 et 1,10 % pT4. Le taux du PSA, six mois après la prostatectomie radicale, était indétec-table dans 81,32 %. En plus de la prostatectomie radicale, les patients avaient bénéficié d'une hormonothérapie (quatre cas), d'une radiothérapie (neuf cas), d'une radiothérapie-hormonothérapie (deux cas). La mortalité opératoire était nulle. La durée moyenne d'hospitalisation était de huit jours. Des complications à type de lymphocèles (6,60 %), de dysérections (47,25 %), d'incontinence urinaire (32,97 %), de rétention aiguë (2,20 %) ont été notées. La survie à trois ans était de 78,33 % après prostatectomie radicale seule et de 100 % après prostatectomie radicale associée à une radiothérapie et/ou à une hormonothérapie. Conclusion : La prostatectomie radicale a permis de contrô-ler le cancer de la prostate localisé, d'où la nécessité d'un diagnostic précoce. L'adjonction de la radiothérapie et/ou de l'hormonothérapie permet d'optimiser le pronostic. Pour citer cette revue : J. Afr. Cancer 3 (2011).
