Epidemiology of Subarachnoid Hemorrhage, Patterns of Management, and Outcomes in China: A Hospital‐Based Multicenter Prospective Study

Abstract: Aneurysmal rupture is the most common cause of spontaneous SAH in China. The percentage of aneurysm obliteration is still low in China that seems to contribute to long-term mortality. With continued training of specialists, proper allocation of healthcare resources, and establishment of stroke centers, the rate of securing aneurysms is expected to rise.

“…In the past 30 years, mortality associated with aSAH has shown a downward trend in China [9], probably due to improved diagnostic techniques, early aneurysm repair, and intensive medical management. However, data from the CNSR showed that mortality was still high at 28 days, 3, 6, and 12 months, that is, 16.9, 21.2, 23.6, and 24.6%, respectively, for patients with aSAH [8]. Furthermore, we found that 34% of patients with aSAH who survive will not regain full independence, and about 70% will have impaired quality of life.…”

“…Most of the studies used to determine risk factors to date are limited by either being single-center series, including a predominance of patients treated by open micro-neurosurgery, or using a selected patient group randomized to clinical trials [7]. Furthermore, advances in diagnostic and treatment strategies for aSAH have substantially improved the outcomes of patients over the last decade(s); however, previous studies on the epidemiology of stroke in China showed that the mortality and disability after aSAH were still relatively high, leading to a huge burden on individuals, families, and society [8, 9]. Nevertheless, limited studies provide data on clinical characteristics, risk factors, and clinical outcomes of DCI after aSAH in China.…”

“…[9101112] The development of both diagnostic modalities and emergent rescuing capabilities is the most important contributor to this discrepancy. This study is based on nationwide data collected from 32 major neurosurgical centers and represents the largest hospital-based epidemiological analysis of SAH in China.…”

“…An 18-point system was used to assess animals' neurological deficits from six aspects, including spontaneous activity (0-3), symmetry movements of all limbs (0-3), outstretching of forelimbs (0-3), climbing (1-3), body proprioception (1-3), and response to vibrissae touch (1)(2)(3). A minimum score is at least 3 and a maximum score is 18.…”

“…By mainly affecting middle-aged patients and a highest fatality in all stroke subtypes, SAH brings about a huge burden on economy and society [1][2][3]. Plenty of researches have focused on the cerebral vasospasm, which was traditionally deemed as the principal cause of poor prognosis in SAH patients [4][5][6][7][8][9].…”
