Epidemiology, risk factors and prognosis of Interferon alpha induced thyroid disorders. A Prospective Clinical Study

Abstract: Thyroid disorders are common during IFN-α therapy. Previous epidemiological data seem to be underestimated. Important risk factors for IITD development are: female sex, elevated serum TSH concentration (≥2.5 μU/mL), positive TPO-Ab and increased blood velocity in thyroid arteries.

“…Preexisting abnormalities of thyroid organs are also thought to be predictive factors for IIT in hepatitis C patients. Both pretreatment abnormalities in serum TSH levels and the presence of anti-TPO antibodies at baseline are known to be risk factors for IIT ( 6 , 10 , 15 ). Therefore, prescreening for serum TSH and the presence of anti-thyroid antibodies (anti-TPO antibody and anti-Tg antibody) are recommended in the guidance for interferon treatment of hepatitis C patients ( 6 ).…”

“…10,11 Nevertheless these medications result in many adverse effects in the HCV positive people, such as flu-like features, bone marrow effects, symptoms pertaining to CNS and many endocrine disorders. 12 Reported prevalence of thyroid diseases in hepatitis C virus positive population is more than in the normal people. 13 Thyroid disorders are suggested because of drugs such as Interferon alpha.…”

“…The contribution of these problems in the pathophysiology of HT is not fully elucidated, but studies on sibling patients, as well as on monozygotic and dizygotic twins, support a relevant genetic influence on the development of the disease [26]. Environmental factors include: iodine consumption [27], the use of certain drugs, such as α-interferon [28], lithium [29] or immune checkpoint blocking molecules [30] and vitamin D deficiency [32]. A role of Yersinia enterocolitica and Helicobacter pylori infections have been hypothesized in the pathogenesis of thyroid autoimmunity, but only HCV infection, so far, has been associated with an increased risk for the development of Hashimoto's thyroiditis [32].…”
