Epidermoid cyst of the testis: An atypical sonographic appearance

Abstract: Epidermoid cysts are rare. They represent the most common benign tumor of the testis. The sonographic appearances of testicular epidermoid cysts usually include avascular, mostly lamellated, heterogeneous internal echotexture, with hypoechoic and hyperechoic concentric rings, accounting for the typical onion-ring appearance. On MRI, epidermoid cysts show a low-signal-intensity center, with internal concentric rings of alternating high- and low-signal intensity on T2-weighted images, which correlates with the o… Show more

“…[ 5 ] Epidermoid cyst of the genitourinary system is rare. There are many cysts occurring in testicles, [ 6 ] but those occurring in the upper urinary tract, such as the kidney and ureter, are extremely rare. Only 4 cases of epidermoid cyst occurring in the upper urinary tract have been reported worldwide to date (Table 1 ).…”

“…A low signal strength peripheral rim was noted on the T1-and T2-weighted images, which gave a target appearance. Because the lesion is avascular, no improvement in contrast is expected after administration of contrast [11].…”

“…Adrenal rests may also appear tumour‐like; however, they only occur in the context of congenital adrenal hyperplasia allowing for their differentiation on clinical grounds 20 . Another intratesticular mimic of tumour is epidermoid cysts which are usually easy to diagnose due to their morphological features 21,22 . Other rare entities include testicular lipomas, hamartomas and sarcoidosis 23 …”
