Epidermolysis Bullosa Acquisita: Clinical Manifestations, Microscopic Findings, and Surgical Periodontal Therapy. A Case Report

Abstract: These results indicate that gingival tissues may also be involved in EBA. Uneventful wound healing after periodontal surgery in this case suggests that periodontal surgery can be performed in patients with EBA. Moreover, both our clinical and histopathologic findings imply that gingivectomy proves useful in maintaining gingival integrity in these patients. Our data may also suggest that the patients with EBA are highly likely to develop dental caries.

“…4 In adult EBA, gingival and enamel abnormalities were reported in one patient with long-standing disease. 5 In conclusion, our patient presented a severe inflammatory variant of EBA associated with caries of the whole dentition. Early and severe onset of EBA could have an impact on the development of dental caries by several mechanisms.…”

“…The tooth abnormalities due to type VII collagen deficiency investigated in Col7a1_/_ and COL7-rescued humanized mice may be attributed to poorly differentiated ameloblasts [21]. Defective enamel structure may provide a flourishing local environment for cariogenic bacteria by providing more adhesive and colonization potential and being more acid soluble.…”

“…Neutrophilic inflammation, which is minimal in the classic form of EBA and more abundant in the inflammatory form, along with eosinophils and monocytes within the dermis can be seen histologically. [156161] The infiltrate is evident perivascularly, in the interstitium, and around follicles, and can lead to fibrosis in older lesions. Direct immunofluorescence reveals the immune-mediated disease process, with a thick band of IgG and, to a lesser extent, C3 deposited linearly at the BMZ.…”
