Background: Organophosphorus compounds have been widely used for a few decades in agriculture for crop protection and pest control. In India Organophosphorus poisoning is the most common. The objective of our study was to measure the LDH Level in acute organophosphorus poisoning Methods: This study was conducted among minimum 100 patients acute organophosphorus poisoning admitted in Casualty ward, MICU and attending medical ward of JLN hospital, Ajmer during Aug 2018 to July 2020. Serum lactate dehydrogenase estimation by spectrophotometric analysis using Beckman Coalter AU 680 Results: These were significantly higher among deaths on day 1 and 3(810±372.99 and 1027.09±458.26, respectively) in comparison to survivors on day 1 and 3 (538.18±300.42 and 365.19±175.49, respectively). Significant difference was found between mean values of different levels of severity of LDH on day 1 and 3. Conclusion: In conclusion, this study found that Serum LDH can be used as biomarker in diagnosis or stratifying severity of acute OP poisoning, as it is cheap and easily available, especially in developing countries. Serial measurements of serum LDH levels in acute OP poisoning can predict the prognosis. Keywords: LDH, OP poisoning, Severity

Chlorpyrifos is one of the most commonly used organophosphorus insecticide. Many studies showed a relationship between pesticide exposure and occurrence of diabetes. The aim was to assess toxic effect of chloropyrifos on pancrease, its ability to induce diabetes mellitus in adult male rats and evaluate whether this toxic insult is permanent or temporary. Material and methods: 24 adult male albino rats were classified into four groups. Group I: negative control. Group II: positive control gavaged orally with 0.5 ml of corn oil once a day for 8 weeks. Group III: gavaged orally with chlorpyrifos dissolved in corn oil (6.75 mg/Kg b.w /day) for 8 weeks. Group IV: orally gavaged with chlorpyrifos by the same dose as group III for 8 weeks, then left without treatment for another 4 weeks. At the end of the study, body weight of all rats were measured then blood samples were taken from all rats to estimate serum levels of amylase, lipase, glucose, insulin, reduced glutathione (GSH) and malondialdehyde (MDA) levels. Then the pancreatic tissues were dissected and submitted to histopathological and ultra-structural examination. Results: chloropyrifos administration caused significant increases in serum amylase, lipase, glucose and MDA with also significant decreament in serum insulin and GSH. These biochemical changes were associated with hisopathological changes. Stoppage of exposure for 4 weeks caused partial to complete improvement. Conclusion: chloropyrifos had toxic effects on pancrease and was considered as a risk factor for diabetes mellitus and improvement could occur after stoppage of its exposure for a period.

Objectives: Organophosphorus compound (OPC) poisoning is primarily a problem in developing countries. Its widespread use and easy availability have increased the likelihood of poisoning with these compounds. Serum cholinesterase levels are easier to estimate and usually depressed after organophosphorus (OP) poisoning. Peradeniya OP poisoning scale has not been studied much in the Indian scenario. It could be a simple and effective system to determine the need for ventilatory support early on in the course. Hence, this study was undertaken to assess the severity of OPC poisoning clinically using Peradeniya scoring and by estimating creatine phosphokinase (CPK) and lactate dehydrogenase (LDH) levels. Material and Methods: A prospective observational study was performed among 100 cases of OP poisoning from January 2021 to October 2022. On patient’s fulling, inclusion criteria were investigated with serum acetylcholinesterase at the time of admission, and periodic estimation of serum CPK and serum LDH was done on the day of admission and then on day three and day 5 of admission Results: The CPK and LDH levels were significantly elevated in patients who developed respiratory failure and thus strongly correlated with clinical severity Conclusion: Serum CPK and LDH show a strong degree of positive correlation with the severity of poisoning and can be used as a predictor of outcome in OP poisoning.
