Escitalopram: Superior to Citalopram or a Chiral Chimera?

Abstract: Background: Escitalopram is the active isomer of the antidepressant citalopram. In theory single-isomer drugs may be superior but few have been found to have clinically significant advantages. The manufacturer claims that escitalopram has more efficacy and a faster onset of effect than citalopram. The purpose of this study was to assess how far these claims are justified. Methods: Relevant trial reports were requested from H. Lundbeck A/S and the Swedish drug regulatory authority. The trials consisted of a poo… Show more

“…22,59 Differences between the two SSRIs seem to depend on the initial severity of the depressive symptomatology, given that escitalopram has shown superior antidepressive efficacy in severely depressed patients. 60,70 Nevertheless, opposite findings were also reported, suggesting methodologic flaws as a cause for the difference in efficacy between the two drugs. 60 Efficacy scores for newer antidepressants are presented in Table 1.…”

“…De tels résultats ne devraient pas être acceptés sans revue de la publication intégrale [18]. Des présentations sélectives de ré-sultats favorables, voire même des conclusions fausses, se glissent parfois dans les dossiers soumis aux autorités de réglementation pour approbation [19]. Finalement les biais de publication représentent un problème majeur pour qui veut pratiquer une médecine fondée sur les évidences (EBM).…”

“…The article of Svensson and Mansfield [26]draws our attention to a relatively new trend in psychopharmacology – the introduction of single isomer drugs extracted from the racemic compounds, chiral drugs. Theoretically, single isomers may be superior to the original compounds, but few have been found to have clinically significant advantages.…”
