Esophageal Reflux Disease Proton Pump Inhibitor Therapy Impact on Sleep Disturbance

Abstract: The existing evidence supports the use of proton pump inhibitors as a treatment for esophageal reflux disease to improve quality-of-life sleep disturbance-related outcomes. Given the wide variability in proton pump inhibitor treatments and sleep disturbance-related outcomes reported, however, study-specific results cannot be directly compared or aggregated. This conclusion appears robust not only for 7 of 8 studies included but also for the 3 highest quality studies.

“…Several placebo‐controlled studies have shown that PPIs improve sleep disturbance as well as nighttime heartburn in patients with GERD . A systematic review confirmed that PPIs improved sleep quality and work productivity . Thus, nighttime heartburn has a significant impact on patients’ sleep quality as well as QoL.…”

“…In addition, it is unclear how acid reflux events affect sleep disturbance without reflux symptoms. Although several placebo‐controlled PPI trials on sleep disturbance showed significant improvement in sleep‐related symptoms and patients’ QoL, polysomnographic parameters, such as total sleep time, sleep onset latency, sleep efficacy, or arousal index, were not changed . It is unclear how PPIs improve sleep‐related symptoms without changing sleep parameters.…”

“…Proton pump inhibitor (PPI) therapy is very effective for controlling reflux symptoms in patients with GERD and so might be expected to help sleep disorder related to this disease. Two systematic reviews of randomised controlled trials (RCTs) of PPI therapy have confirmed that acid suppression improves sleep quality. The most recent of these identified eight RCTs that have addressed the impact of PPI therapy on sleep.…”

“…Two systematic reviews of randomised controlled trials (RCTs) of PPI therapy have confirmed that acid suppression improves sleep quality. The most recent of these identified eight RCTs that have addressed the impact of PPI therapy on sleep. Only three of these studies had a large sample size and all trials predominantly evaluated patients in secondary care and all compared PPI with placebo.…”
