Establishing a Contextually Appropriate Laparoscopic Program in Resource-restricted Environments

Abstract: A contextually appropriate long-term partnership may assist with laparoscopic upskilling of colleagues in low- and middle-income countries. This type of collaboration promotes local ownership and may translate into better patient outcomes associated with laparoscopic surgery. In resource-restricted environments, the factors threatening sustainability may differ from those in high-income countries and should be identified and addressed.

“…In these countries, the challenges posed by the burden of primary healthcare concerns have limited government support for development of modern tertiary healthcare facilities, and laparoscopic surgery is practiced in only a few tertiary hospitals. Innovative programs to train surgeons and develop low-cost equipment in these countries are encouraging [63]. Some studies in the literature have focused on the feasibility of implementing laparoscopic procedures in resource-poor countries, and how to overcome the challenges involved [6466].…”

“…The second, classic model of neurosurgery-related international aid is one in which surgeons or surgical teams make short trips to perform surgery and teach techniques to local surgeons [11][12][13]. In some cases, participants visit the same location multiple times, which has the added advantage of longitudinal follow-up and monitoring of the initiative's impact.…”
