Estimation of Fetal Weight in Fetuses With Abdominal Wall Defects

Abstract: None of the 3 sonographic formulas is ideal for estimating fetal weight in fetuses with AWDs. The Siemer formula should be used when accuracy in the absolute EFW is the goal. For the purpose of making the more clinically relevant diagnosis of IUGR, use of the Hadlock formula is justified.

“…One strength of our study is the relatively large sample size when compared to similar studies looking at accuracy of sonographic formulae in estimating weight in fetuses with AWDs. Nicholas et al 22 compared two different sonographic formulae to the Hadlock formula in accuracy of EFW and efficiency of screening for fetal growth restriction (FGR). Like our study, this was also a retrospective cohort study and included 76 fetuses (53 with gastroschisis and 23 with omphalocoele), and EFW was calculated using the Honarvar formula (using FL alone), Siemer’s formula as previously described, and the Hadlock formula.…”

“…To assess estimated fetal weight in these fetuses can be difficult by the hernia as measurement of abdominal circumference may be inaccurate. ough some different algorithms have been proposed, we used the Hadlock formula placing the calipers around the abdominal wall and avoiding the hernia so that the ultrasound measuring tool drew the estimated circumference without the omphalocele [18]. All schedules were performed by the same two sonographers to minimize measurement mismatch.…”
