“…In this table, the Standardized Residual Correlation (SRC) value is presented to measure the mismatch of the scale of five and four response categories in the ELVI instrument (Maydeu-olivares et al, 2017). SRC values are based on item correlation values from negative to positive ranges which are then compared with significance values (p <α = 0.05) (Gonza et al, 2014).Rasch modeling with the Winsteps program version 4.0.1 obtained SRC value of items -0.177 to 0.638 on scale of five response categories and -0.195 to 0.565 on scale of four response categories. This article discusses the psychometric validity of the Indonesian version of emotional learning instruments with a scale of five and four response categories. The purpose of this study is to produce an Indonesian version of emotional learning instrument with an effective response category scale used by Indonesians. The instrument is a modification of the scale of the Learning Environment Research Questionnaire on Emotional Climate Classroom. This study is a survey of 1494 responses of 7th and 8th grade junior high school students. Samples were selected by random sampling and based on considerations of schools implementing the 2013 Curriculum. Modification instruments consisting of 43 items were tested in obtaining validity based on item difficulty estimations and psychometric criteria with Rasch modeling. The results of this study indicate that the Andrich threshold validity testing meets the monotonic characteristics and the Standardized Residual Correlation is higher, so the scale of the five response categories is more effective to measure the Indonesian version of emotional learning instruments than the scale of the four response categories. Objectives: to understand the perceptions of ethics and bioethics and how to be ethical and bioethical in daily life of Primary Health Care, from the perspective of nurses. Methods: this is a Holistic-qualitative Multiple Case Study, based on Comprehensive Everyday Sociology, with 54 participants. Results: two subcategories and the category Being ethical and bioethical in daily life of PHC: nurses’ perceptions emerged. The ethical and bioethical being permeates a subjective and abstract self, whose fears, anxieties and concerns are intertwined with the human and professional dimensions in daily work and in personal-professional relationship. Ethics and bioethics perceptions emerge from subjectivity, established relationships, lived experiences and daily actions of nurses essential to the profession, professionals and individuals to be cared for. Final Considerations: ethical and bioethical perceptions and attitudes are essential to care, management and organizational actions, health care, and the safety of users and professionals.

Background Recognizing and appropriately responding to ethical considerations is a crucial element of ethical nursing practice. To mitigate instances of ethical incongruity in healthcare and to promote nurses’ comprehension of their professional ethical responsibilities, it is imperative for researchers to accurately evaluate ethical sensitivity. Conducting a systematic review of the available instruments would enable practitioners to determine the most suitable instrument for implementation in the field of nursing. Aim This review aims to systematically assess the measurement properties of instruments used to measure ethical sensitivity in nursing. Methods A systematic literature search was conducted in July 2022 in the following electronic databases: Scopus, CINAHL, APAPsycINFO, Embase, Web of Science, and PubMed. Two reviewers independently screened and assessed the studies in accordance with the COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN) checklist. The updated criteria for good measurement properties are used to rate the result of measurement properties, and the modified Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach was used to grade the quality of the summarized evidence. Results This review encompasses a total of 29 studies that describe 11 different instruments. Neither cross-cultural validity nor responsiveness was examined in any of the included studies. Whereas the majority of the instruments were conducted with at least some type of validity assessment, nearly all of the reliability results rated were indeterminate. Two instruments were recommended, the Ethical Sensitivity Questionnaire for Nursing Students (ESQ-NS) and the Ethical Awareness Scale for nurses in intensive care units. It is recommended that new self-administration instruments for special nursing settings be developed in accordance with the item response theory (IRT)/Rasch model. Conclusion The selection of ethical sensitivity measurement instruments in nursing, and further research on the development, psychometric, and cross-cultural adaptation of these instruments, could be conducted in accordance with the findings and suggestions of this systematic review. Strengths and limitations • This review was conducted to assess 11 instruments that were used to measure ethical sensitivity in nursing in 29 studies. • The Ethical Sensitivity Questionnaire for Nursing Students (ESQ-NS) and the Ethical Awareness Scale for nurses in intensive care units can be recommended, but further reliability and cross-cultural validity testing are needed. • The IRT/Rasch model is also recommended to measure ethical sensitivity in nursing. • The potential limitation of utilizing the COSMIN checklist for assessing methodological quality is worth considering. • Test–retest was considered inappropriate; thus, the reliability testing of ethical sensitivity measurement instruments still needs to be explored.

Abstract: Paternalistic behaviour persists in PHC. Lesser endorsement of items suggests that patient-centred care and patient autonomy are not fully considered by professionals. Ethical sensitivity could improve if patients are cared for by multidisciplinary teams.
