Etretinate Treatment in Disseminated Porokeratosis

Abstract: A 77‐year‐old man with disseminated superficial porokeratosis of Mibelli (PM) over the whole body surface for 60 years was treated with oral etretinate (1 mg/kg/day) for nine months after resection of two lesions of Bowen's disease. Clinically, hyperkeratotic plaques were flattened after three months of treatment. Histological changes included disappearance of parakeratosis and residual but less stacked cornoid lamella. Etretinate may be the first choice of treatment in widespread porokeratosis.

“…Oral etretinate has been reported to be beneficial in the treatment of porokeratosis, especially in the disseminated form of the disease 13–16 . However, as far as we reviewed the published work, there was no report that stated the effect of oral etretinate for the hyperkeratotic variant of porokeratosis.…”

“…11 Oral etretinate has been reported to be beneficial in the treatment of porokeratosis, especially in the disseminated form of the disease. [13][14][15][16] However, as far as we reviewed the published work, there was no report that stated the effect of oral etretinate for the hyperkeratotic variant of porokeratosis. In our case, oral administration of etretinate brought about a remarkable improvement in the verrucous nodule on the third toe of the left foot and a gradual improvement in the multiple hyperkeratotic papules on the lower extremities and hyperkeratotic plaques on the buttocks.…”

“…Other therapies that have been tried in DSAP are topical 5-fluorouracil, retinoids, calcipotriol, cryotherapy, systemic retinoids and corticosteroids [13, 14, 15, 16]in other subtypes of porokeratosis. Spontaneous and complete regression of the lesions, after a change in the immunological status of the patient, has also been reported [7, 8, 13].…”

“…Pruritus is not unusual. Therapeutic alternatives, such as topical 5-fluorouracil, retinoids, calcipotriol, cryotherapy, systemic treatment with retinoids or corticosteroids, have been reported by different authors [13, 14, 15, 16]. Histopathological findings always show typical presence of a cornoid lamella, but the association with subepidermal blisters (bullous variant) has never been described before.…”
