EUS-guided ethanol ablation therapy for gastric stromal tumors

Abstract: EUS-guided fine-needle ethanol injection (FNI) therapy of some types of cystic and solid tumors has been documented. However, reported cases to date of gastrointestinal-stromal tumors (GIST) treated with this technique are scarce. Ethanol ablation is an alternative treatment with a low rate of adverse events in selected cases.

“…In subepithelial gastrointestinal tumors (GIST), case reports of endosonographic fine needle injections (FNI) for ablation with ethanol[ 41 ] and RFA[ 42 ] have been successful in cases where patients either rejected or had prohibitive comorbidities to surgical resection. In this class of tumors, appropriately selected neoadjuvant and/or adjuvant tyrosine kinase inhibitors tend to have a favorable effect in disease control as well[ 15 ].…”

“… 119 Such direct comparison work has largely been abandoned and understudied, with widespread adoption of RFA and MWA for HCC. Other less common applications of PEI include ablating pancreatic tumors, 120 - 122 gastrointestinal stromal tumors, 123 and ablating the vascular supply to thyroid and parathyroid tumors. 124 , 125 Overall, PEI treatment has mild complications and adverse effects, which are caused by leakage of ethanol into surrounding tissues or organs.…”

“…The amount of tissue required to make the GIST diagnosis is 1 cm 3 (about 5–10 core-needle biopsy specimens) [ 5 ]. EUS (endoscopic ultrasound)-guided fine-needle with 95% ethanol injection (FNI) therapy of GISTs in adults is an effective treatment method [ 87 , 88 ], however, its application in pediatric GISTs has not been set down yet.…”
