The Underlying cause of death is an event or condition without which the patient would not have died. The aim of the research is to determine the accuracy of writing the sequence of causes of perinatal death based on mortality rules The research method used in this research is descriptive research, with a retrospective approach. Data collection using observation and interviews. The research population was 50 medical record documents of patients who died perinatally in 2018-2022 using a saturated sampling technique. The percentage of accuracy in writing the sequence of perinatal deaths at PKU Muhammadiyah Hospital Surakarta shows 100% inaccurate. The writing inaccuracy is due to the writing not being in accordance with the existing standart Operating Procedure, the order of perinatal death is still written on the medical certificate for the cause of adult death. The accuracy of writing the order of perinatal deaths is still relatively low. It would be better if the hospital further increases its outreach activities so that medical personnel can write the order of death on the certificate correctly in accordance with the rules or standards that apply in the hospital.

Coding accuracy, namely the process of conformity of the diagnosis code that has been set by the coding officer based on ICD-10 which greatly affects data reporting and administration. The basic cause of death is the course of any disease, sick condition, or injury that causes or causes the accident that causes death. The purpose of this study was to determine the accuracy of the underlying cause of death code based on the selection rule and the MMDS table in patients who died at the Duren Sawit Hospital in 2022. This study used a descriptive method with a quantitative approach which took 88 samples using a saturated sample technique by means of observation and interviews. The results of the study were obtained from 88 samples of the accuracy of the basic causeof death codes based on the general principle selection rule and rule 1 at the Duren Sawit RSKD found that 49 (56%) and 39 (44%) were incorrect. There are factors that affect the inaccuracy of using the 5M elements (Man, Money, Material, method, Machine), namely the man element because the coding officer's profession is not appropriate and less thorough and the elements of the general coding SPO method which are still being revised, do not use selection rules and MMDS tables. Suggestions that officers should be given socializationabout the selection rules and MMDS tables.

“…Kode yang tidak sesuai dalam ICD-10 CM dipengaruhi oleh pengetahuan koder bahwa ditemukan koder tidak teliti dalam memeriksa dan memberikan kode penyebab dasar kematian (Widyaningrum and Kuntari, 2016). Adanya petugas yang belum mengetahui penggunaan ICD-10 dan tabel MMDS (Rahmawati and Lestari, 2018), kurangnya komunikasi antar tenaga medis ketika diagnosis tidak dapat terbaca dan tidak lengkap (Mardiawati and Akika, 2019), dan tidak adanya audit coding untuk mengevaluasi ketepatan sebab dasar kematian kode yang telah diberikan (Nuryati and Hidayat, 2014) dan beberapa petugas tidak mengikuti pelatihan atau seminar koding khusus kode sebab dasar kematian (Hardono and Ernawati, 2018). Ketersediaan Standar Operasional Prosedur untuk sebab dasar kematian tidak ada sehingga koder hanya melakukan pengodean dari diagnosis yang telah ditulis oleh dokter (Ilmi and Purbobinuko, 2020).…”

Pelaporan mortalitas dilakukan sebagai upaya untuk pencegahan penyakit yang mematikan dan sebagai evaluasi fasilitas pelayanan kesehatan. Dalam memilih kode pada sertifikat medis penyebab kematian perlu diperhatikan agar pelaporan dapat terlaksana secara optimal. Kode yang tepat adalah kode yang sesuai dengan ICD-10 serta dibantu dengan Tabel MMDS. Tujuan dari review ini melihat melihat ketepatan kode diagnosis penyebab dasar kematian dan untuk mengetahui faktor penyebab ketidaktepatan pengodean diagnosis dan pengisian sertifikat medis penyebab kematian di rumah sakit. Metode penelitian ini menggunakan metode literatur review terhadap sejumlah artikel penelitian yang dipublikasikan rentang waktu tahun 2011-2021 dan ditemukan sebanyak 13 artikel jurnal memenuhi kriteria penelitian. Hasil menunjukkan presentasi ketepatan kode diagnosis sebesar 83% dan presentase ketidaktepatan kode diagnosis sebesar 90%. Hasil studi literatur ini juga membahas faktor penyebab ketidaktepatan pengodean diagnosis dan pengisian sertifikat penyebab dasar kematian yaitu faktor Man, Method, Material, Machine, dan Money.
