Evaluation of desensitization protocols to betalactam antibiotics

Abstract: What is known and objective Betalactam antibiotics are the most frequent cause of hypersensitivity reactions. Rapid drug desensitization (RDD) is a technique that induces temporary tolerance to a drug allowing a patient to receive the optimal agent. The increased use of RDD and the lack of standardization among available protocols in terms of formulation, starting dose, number of steps and dosing frequency make it essential to determine the safety and appropriate management of these protocols, especially regar… Show more

“…The studies carried out so far have been mainly descriptive with small sample sizes ( González-García et al, 2021 ; Rodríguez-Alarcón et al, 2022 ). Based on current evidence, desensitization should be used as a last resort in the management of PAL patients.…”

“…A high comorbidity index and chronic kidney disease did not induce antibiotic-related adverse events in the study group, which is an important finding because patients with renal insufficiency, who cannot take certain alternative treatments due to their nephrotoxicity, may be a suitable group for desensitization. “…67 Various RDD protocols with different BL drugs, both oral and intravenous, have been found to be safe and effective. 67,69 In conclusion, this procedure allows for the maintenance of first-line antibiotics, leading to greater efficacy, fewer side effects, and a longer life expectancy for patients compared to the use of second-line therapies.…”
