Evaluation of different confirmatory algorithms using seven treponemal tests on Architect Syphilis TP-positive/RPR-negative sera

Abstract: The Architect Syphilis TP is considered to be a suitable screening test due to its high sensitivity and full automation. According to the International Union against Sexually Transmitted Infections (IUSTI) 2014 guidelines, however, positive screening tests need confirmation with Treponema pallidum particle agglutination (TP.PA). Among Architect-positive results, samples with a negative non-treponemal test present the major diagnostic challenge. In this multicenter study, we investigated if other, preferable le… Show more

“…Jonckheere et al . [ 16 ] tested 178 RPR negative samples using Architect at S/CO values ranged between 1 and 15. Based on TP-PA confirmatory assay, specimens with S/CO > 5.6 were considered positive and do not require further confirmation testing as compared to the higher S/CO threshold of 16.4 predicted in our study.…”

“…Our retrospective data add to the body of evidence that the screening titer for RPR and quantitative antibody index values for syphilis IgG correlate with the probability of confirmation [4] , [7] , [8] , [23] , [24] . The higher the RPR titer and syphilis IgG value, the more likely a patient is to confirm with subsequent testing.…”

“…This test has shown perfect performance when compared to the TPPA 15 and could be used as a reliable test for the detection of syphilis. 16,17 The results of the chemiluminescent reaction are measured in relative light units (RLUs), with an RLU value of <1.00 indicating a negative result and an RLU value of 1.00 indicating a positive result. The level of RLUs is directly related to the amount of anti-T. pallidum antibodies in the serum.…”
