Evaluation of the awareness, control and cost-effectiveness of hypertension treatment in a Brazilian city: populational study

Abstract: Approximately half of the participants received monotherapy. The best percentage of control with monotherapy was obtained with beta-blockers but the diuretics treatment was the most cost-effective. The levels of awareness and control were high compared with developed countries, most evident in the higher social classes and higher education levels.

“…The prevalence of hypertension based on BP measurement or antihypertensive drug use was 43.4% (95%CI: 39.1 -47.7), while the prevalence of self-reported hypertension was 41.9% (95% CI: 37.7 -46.2) ( Table 1). This study sample was similar to the original sample as regards gender, age, income and level of education ( Validation studies on self-reported hypertension in Brazil indicate sensitivity from 51% to 84% [10][11][12][13][14] . However, data regarding the population of the city of São Paulo is not available.…”

“…Another important factor involved in the occurrence of HC is the rate of hypertensive control. In a study of a Brazilian population, Moreira et al 14 showed that the percentage of BP control among all hypertensive individuals was 38.6%. Among treated hypertensive patients, this percentage was 52.9% and was even higher in the population with more years of schooling, which also was of a higher socioeconomic class.…”

“…Although world data estimates suggest that there is no difference in hypertension prevalence between genders, 1 most previously published studies found a higher prevalence of HC among women. 2,[4][5][6] This statistic could mean that women are more aware of their condition and more frequently seek primary care, 14 despite the fact that males tend to have more serious complications related to hypertension, as evidenced by the higher rate of HE presentation found among males. 2,6,15 Despite the slightly higher prevalence of HC in women, our results suggest that this prevalence represents an underestimation.…”

“…3 More than 60% of patients with hypertension do not achieve adequate disease control. 4,5 The high prevalence of patients who do not achieve adequate disease control increases the occurrence of negative clinical outcomes, such as hospitalizations, disease complications (e.g., retinopathy, nephropathy, neuropathy, stroke, and acute myocardial infarction), earlier retirement caused by disease complications, and death. [6][7][8][9][10][11] The negative clinical outcomes of diabetes mellitus and hypertension cause increases in direct and indirect economic costs to society.…”
