Evaluation of the response to treatment of psoriasis vulgaris with reflectance confocal microscopy

Abstract: This is the first study with a large group of patients to perform a noninvasive assessment with RCM of the response of psoriasis to different treatments: phototherapy, systemic and topical treatment. Micrometric and morphometric changes occurring in the psoriatic tissue during the 8-week treatment period were identified by in vivo RCM in a noninvasive manner. RCM is capable of monitoring of treatment response in psoriasis.

“…After reviewing all publications, an overview of general psoriatic RCM features was provided for diagnostic and monitoring purposes (Table ). Four therapeutic follow‐up studies monitored different therapeutics; phototherapy [ultraviolet B (UVB) and psoralen ultraviolet A], topical non‐steroidal anti‐inflammatory drugs, topical corticosteroids, Methotrexate, Acitretin and/or Calcipotriol and Adalimumab by RCM . RCM features showed a good correlation with clinical improvement and improvement of Psoriasis Area Severity Index (PASI) scores .…”

RCM features showed a good correlation with clinical improvement and improvement of Psoriasis Area Severity Index (PASI) scores . In addition, RCM was able to show subclinical psoriatic features (orthokeratosis with abnormal compact aspect, high number of dermal papillae (DP), vascularization in DP and drastic reduction in inflammation) and in non‐responders to UVB large clustered tortuous capillaries were shown .…”
