Even mild cases of paediatric Henoch‐Schönlein purpura nephritis show significant long‐term proteinuria

Abstract: Even mild forms of HSP nephritis risk significant long-term proteinuria. Very early introduction of ACEi/ARB may improve the long-term outcome independent of histological lesions.

“…The patients were diagnosed with HSP according to the European League Against Rheumatism/Pediatric Rheumatology International Trials Organization/Pediatric Rheumatology European Society criteria, which include the presence of purpura or petechiae (mandatory) with lower-limb predominance plus one of the following four conditions: abdominal pain, histopathology, arthritis or arthralgia, or renal involvement [29]. Of 32 patients with HSPN enrolled in this study, 21 nephrotic syndrome (urine protein/creatinine ratio >220 mg/mmol or 24-h protein >50 mg/kg/24 h) or persistent proteinuria for at least 3 months underwent renal biopsy [11,38]. The histological grade was assessed according to the International Study of Kidney Disease in Children criteria.…”

“…İncelediğimiz çalışmalarda hastaların % 29'una steroid tedavisi verilmiş idi. Spontan remisyon ihtimali gözardı edilmemek kaydı ile, özel-likle devam eden proteinürisi bulunan hastalarda, tedavide Anjiotensin dönüştürücü enzim inhibitörleri (ACEİ) ve anjiotensin reseptör blokerlerinin (ARB) renal prognoz üzerinde olumlu etkileri bulunduğu bildirilmiştir 33 . Dirençli HSP nefritlerinde siklofosfamit, mikofenolat, azatioprin, siklosporin A, rituximab gibi tedavi yöntemleri denenmiş, olumlu yanıtlar alınmıştır 3,34,35 .…”

“…Narchi et al, [10] detected that renal manifestations occurred within four weeks in 84%, within six weeks in 91% and within six months in 97% of the patients and therefore they suggested that a six weeks of follow up will be sufficient. In a recent study of Tudorache E et al, [11] the authors found that even mild HSP nephritis had been associated with long term proteinuria. Ronkainen et al, [9] showed that 72% of patients in their cohort had renal symptoms after a mean follow-up of 19 years.…”
