Evidence review and ENT‐UK consensus report for the use of aminoglycoside‐containing ear drops in the presence of an open middle ear

Abstract: ENT-UK recommends that when treating a patient with a discharging ear, in whom there is a perforation or patent grommet: if a topical aminoglycoside is used, this should only be in the presence of obvious infection. Topical aminoglycosides should be used for no longer than 2 weeks. The justification for using topical aminoglycosides should be explained to the patient. Baseline audiometry should be performed, if possible or practical, before treatment with topical aminoglycosides.

“…Quinolone and aminoglycoside preparations are commonly used and have similar effectiveness [15]. Aminoglycosides should only be prescribed in short courses and when there is obvious activity in mucosal COM to reduce the theoretical risk ototoxicity [16]. Quinolone preparations do not carry a risk of ototoxicity.…”

“…A total of 15 patients had bilateral disease, of which only five patients were prescribed separate topical antibiotic bottles for each ear. This study demonstrates the wide variation in practice in the management of otitis externa and highlights the confusion over the use of topical and oral antibiotics [4].…”

“…Furthermore, concern about ototoxicity associated with eardrops and limited availability of national guidelines in the UK have caused much confusion amongst GPs over the use of topical and oral antibiotics [4,5].…”

“…ENT-UK, the British association for otorhinolaryngologists, advise that aminoglycoside drops can be used in the presence of a perforation for a maximum of 2 weeks provided there is an obvious infection. 9 This is because pus in the middle ear is in itself ototoxic. The justification should be explained to the patient and baseline audiometry performed if practical.…”
