Background Preclinical and clinical investigations have revealed deficits in cortical inhibition in individuals with schizophrenia. Transcranial magnetic stimulation, a commonly used noninvasive measurement technique, is used for assessing these deficits. Limited research has been conducted on the effects of antipsychotic medications on cortical inhibition. This study aimed to evaluate the effects of clozapine on cortical inhibition with transcranial magnetic stimulation longitudinally and compare it with unaffected controls. Methods Ten patients who started clozapine were assessed at baseline, with 8 reassessed after 4 months. Eight age- and sex-matched unaffected controls were included. Psychopathology, neurocognitive performance, formal thought disorder, and disability were assessed, and the cortical excitability parameters (resting motor threshold, cortical silent period, short-interval intracortical inhibition, intracortical facilitation, and short-latency afferent inhibition [SAI]) were measured at baseline and four months after clozapine treatment. Results Resting motor threshold, ICF, and SAI were significantly different between patients and controls at baseline, whereas resting motor threshold, SAI, and ICF became similar to controls after clozapine with only ICF having a trend for significance. Clozapine prolonged cortical silent period significantly in the patients. Conclusions This is the first study to investigate the effect of clozapine on SAI, a potential cholinergic biomarker, and the first follow-up study to investigate the relationship between the effects of clozapine on cortical inhibition and cognition. Clozapine seems to cause an increase in cortical inhibition through GABAergic and possibly cholinergic mechanisms. However, additional follow-up studies with larger sample sizes are required to reach more robust conclusions.

Objectives We aimed to investigate the characteristics of adolescents with Bipolar disorder-I with irritability and agitation (Mania+IA) compared to those without irritability and agitation (Mania-IA) in a multi-center representative sample. Methods Data of 145 patients from three tertiary-care inpatient units between 2016 and 2021 were obtained. Psychomotor agitation was defined as a score of ≥3 on the YMRS “Increased Motor Activity––Energy” item, irritability as a score of ≥4 on the YMRS ‘irritability’ item, and severity anchors of speech and thought disturbance on the YMRS ‘6 and 7’ items. Results Previous manic episodes ( p = 0.013), involuntary hospitalization ( p = 0.006), psychotic features ( p = 0.001), formal thought disorder ( p = 0.010) and aggressive/disruptive behavior ( p = 0.021) were more frequent in the Mania+IA group. Conversely, depressive episodes ( p = 0.006) and family history of depression ( p = 0.024) were more frequent in the Mania-IA group. The Mania+IA had poorer functioning at the time of discharge. Conclusions Irritability and agitation were closely related to complications, psychotic symptoms and thought disorder. Assessment and monitoring of psychomotor agitation and irritability may help child and adolescent psychiatrists to predict clinical difficulties and appropriate interventions.

Şizofreni ve ikiuçlu duygudurum bozukluğunun (İUB) akut hastalık döneminde gözlenen düşünce süreci bozukluklarının (DSB) karşılaştırılması ve mani ile ilişkili DSB boyutlarının belirlenmesi amaçlanmıştır.Yöntem: Araştırmaya standartlaştırılmış düzelme (remisyon) ölçütlerini sağlamayan şizofreni tanılı katılımcılar (n=34) ile İUB mani döneminde olan katılımcılar (n=20) dahil edildi. Katılımcılara Pozitif ve Negatif Sendrom Ölçeği, Klinik Global İzlenim Ölçeği, Hamilton Depresyon Derecelendirme Ölçeği ve Young Mani Derecelendirme Ölçeği, DSB için Düşünce ve Dil Bozukluğu Ölçeği (Thought and Language Disorder Scale-TALD) uygulandı. Düşünce süreci bozukluklarının tanı ile ilişkisi TALD faktörleri ve tanı gruplarını içeren lojistik regresyon modeli ile değerlendirildi.Bulgular: Şizofreni ve İUB grubunda demografik özellikler, global hastalık şiddeti ve TALD'ın nesnel pozitif faktör puanı arasında fark saptanmadı. Ancak nesnel negatif ve öznel negatif faktör puanı şizofreni grubunda daha yüksekken, öznel pozitif faktör puanı İUB grubunda daha yüksek saptandı. Regresyon modelinde öznel pozitif faktör İUB tanısı ile, nesnel negatif faktör şizofreni ile ilişkiliydi. İUB grubunda ilişki çözümlemesinde mani şiddetinin TALD puanıyla pozitif yönde korelasyon gösterdiği, hastalık süresinin nesnel pozitif faktör ile pozitif yönde, öznel negatif ve öznel pozitif faktörle negatif yönde korelasyon gösterdiği saptandı.Sonuç: Bulgular nesnel pozitif DSB'nin akut hastalık döneminde şizofreni ve İUB'de ortak olduğunu, ancak öznel pozitif DSB belirtileri ile nesnel negatif DSB'nin şizofreniyi İUB mani döneminden ayırmaya yardımcı olabileceğini göstermektedir.Anahtar Kelimeler: Düşünce süreci bozuklukları, şizofreni, ikiuçlu duygudurum bozukluğu mani dönemi SUMMARY Comparison of Formal Thought Disorder in the Acute Episode of Schizophrenia and Manic Episode of Bipolar Affective DisorderObjective: The aim of this study was to compare the formal thought disorder (FTD) in the acute episode of schizophrenia (SCHZ) and bipolar affective disorder (BPAD), and to determine the FTD dimensions associated with BPAD. Method:The study included a total of 34 SCHZ patients not meeting the standardized remission criteria and 20 patients in BPAD manic episode. The patients completed the Positive and Negative Syndrome Scale (PANSS), the Clinical Global Impression Scale (CGI), the Young Mania Rating Scale (YMRS), the Hamilton Depression Rating Scale (HAM-D) and the Thought and Language Disorder Scale (TALD) in order to assess FTD. The association of FTD with the diagnoses was analyzed by a logistic regression model including the TALD factors and the SCHZ and BPAD groups.Results: Statistically significant differences were not determined between the demographic features, the CGI scores and the TALD objective positive factor scores of the SCHZ and BPAD groups.
