Exercise-Induced Anaphylactic Syndromes

Abstract: To differentiate the diagnoses of exercise-induced anaphylaxis and cholinergic urticaria/anaphylaxis, we developed reproducible diagnostic provocative challenges. The data derived from the study of two representative patients, one with cholinergic urticaria and the other with exercise-induced anaphylaxis, suggest approaches to distinguishing these diagnoses. After specific exercise challenges, both patients developed symptoms consistent with anaphylaxis and had associated increases in plasma histamine levels. … Show more

“…Exercise is associated with significant alterations of endocrine (Irvine 1982) and metabolic parameters (Snow 1979). Disorders specifically linked with onset of exercise have been described in man (Casale et al 1986) and horses (Logan et al 1992). The potential role for a physiological lowering of the threshold for photic headshaking with exercise remains to be investigated.…”

“…In addition, exercise-induced histamine release has been demonstrated in exercise-induced urticaria and anaphylaxis (8). Moreover, there was a marked increase in the wheal response to compound 48/80 (a cutaneous mast cell secretagogue) and not to histamine after a combination of food and exercise challenge in patients with food-associated exercise-induced urticaria-angioedema (9).…”

“…Although EIA used to be reported as a component of severe cholinergic urticaria, 9 it appears to be a distinct entity 10,11 . EIA can be distinguished from cholinergic urticaria by passive heat challenge 11 . After a passive heat challenge that increases core body temperature by more than 0·7 °C, only patients with cholinergic urticaria develop an anaphylactic reaction.…”
