Expansion of CMV-specific CD8+CD45RA+CD27- T cells in B-cell chronic lymphocytic leukemia

Abstract: In patients with B-cell chronic lymphocytic leukemia (B-CLL), the absolute number of T cells is increased. Although it has been suggested that these T cells might be tumor specific, concrete evidence for this hypothesis is lacking. We performed a detailed immunophenotypic analysis of the T-cell compartment in the peripheral blood of 28 patients with B-CLL (Rai 0, n ‫؍‬ 12; Rai I-II, n ‫؍‬ 10; Rai III-IV, n ‫؍‬ 6) and 12 healthy age-matched controls and measured the ability of these patients to mount specific i… Show more

“…The above findings suggested a connection between the number of virus-specific T cells in latency and their phenotype which would accord with the expansion of CMV-specific CD8 ϩ CD27 Ϫ T cells in immunocompromised patients (11,22), B cell chronic lymphocytic leukemia patients (23), and the elderly (24,25).…”

“…However, there is now interest in the potential health associations of chronic viral carriage in a range of clinical settings, particularly in patients with B‐CLL in whom the CMV‐specific T cell immune response can expand to a remarkably high frequency 15, 16. Despite this, no published epidemiological data exists investigating the relationship between CMV and B‐CLL prognosis.…”

“…The CD4:CD8 ratio is markedly lower in CMV seropositive individuals compared with those who remain uninfected, and this effect is particularly profound in patients with CLL. Indeed, one of the paradoxes of CMV infection in patients with CLL is that the magnitude of the virus‐specific immune response is actually higher than in healthy individuals, despite the state of immune suppression, in a mechanism that is believed to be a response to increased levels of subclinical viral reactivation 15, 16. As such there is now considerable interest in how chronic CMV infection may influence the clinical profile of patients with B‐CLL 17.…”

“…36 These data are in line with observations in B-cell chronic lymphocytic leukemia patients, where an expansion of CD8 þ T cells was related to CMV infection. 37 There also seems to be a potent stimulation of T cell function by subclinical CMV infections. 18 Furthermore, T cell recovery was suggested to be influenced by the anti-viral treatment itself, 38 when ganciclovir prophylaxis and pre-emptive therapy were compared, but these treatment-related observations were not made consistently.…”
