Experience with Arteriovenous Fistulas (CIMINO) in Chronic Hemodialysis

Abstract: Experiences with 45 uremic patients with arteriovenous fistulas for dialysis are described. The operation is easy and successful if the vessels of the patient are intact. Varicose syndromes of the hand may occur but can be managed. Complications from cannulations, such as large hematomas, are rare but may obstruct otherwise good fistulas. The fistula should be constructed in good time before a definite need for dialysis arises.

“…The much lower cost in the case of AVF is also an advantage. However, despite the advantages of AVF, the primary failure rate is in the region of 13–28% [5–7]. A number of reasons for the high failure rate has been discussed in the literature such as unsatisfactory veins, poor flow in artery, faulty surgical technique, age greater than 65 yr and female sex.…”

“…Originally described as side‐to‐side anastomosis between the radial artery and the cephalic vein, it was then adapted to include end‐to‐side, end‐to‐end anastomoses, and other locations. At that time, early fistula failure was reported in 5%‐12% of cases . At its inception, dialysis was performed through the fistula the day after surgery using a tourniquet to produce venous engorgement.…”

[12][13][14] At its inception, dialysis was performed through the fistula the day after surgery using a tourniquet to produce venous engorgement. Its reliability, infrequent infection, and thrombosis, as well as its easy maintenance, made it the ideal option in patients who had suitable vessels.…”
