Experience with Endoluminal Ultrasonography in the Urinary Tract

Abstract: Ultrasonography has been an invaluable tool in the field of urology for its noninvasiveness, safety, and relatively low cost. However, examination of the ureter with ultrasound is difficult because of the distance of the transducer from the ureter and because of intervening structures such as nonconductive bowel gas. As smaller probes have become available, attempts have been made to apply them to endoluminal use. Endoluminal ultrasonography has been employed in urology to examine the proper placement of injec… Show more

“…Many efforts have been made to assess cancer stage and grade preoperatively, including the use of sophisticated biopsy devices and endoluminal ultrasound3,4. However, an aggressive biopsy technique involves a risk of ureteral perforation and has a low success rate; ultrasound does not have sufficient resolution to stage early ureteral cancer.…”

“…IVUS uses a high‐frequency transducer placed on the tip of a catheter and provides a high‐resolution, cross‐sectional image perpendicular to the long axis of the vessels. This technique is commonly used for coronary interventions, but, has been applied recently to the urinary tract, pancreatic duct, bile duct, portal vein, and mesenteric artery to detect tumoral infiltration. IVUS enables clear visualization of the wall of the IVC as a single‐layered echogenic ring, which allows differentiation between compression of the vessel wall and tumor invasion.…”

“…When endoscopic techniques are employed for UPJ obstruction, endoluminal ultrasound has proven a valuable adjunct to better map the anatomy and diagnose crossing vessels before cautery or laser endopyelotomy. 9 The use of electrosurgical devices has been replaced by holmium laser in our practice.…”
