Expert-independent classification of mature B-cell neoplasms using standardized flow cytometry: a multicentric study

Abstract: Reproducible expert-independent flow-cytometric criteria for the differential diagnoses between mature B-cell neoplasms are lacking. We developed an algorithm-driven classification for these lymphomas by flow cytometry and compared it to the WHO gold standard diagnosis. Overall, 662 samples from 662 patients representing nine disease categories were analyzed at 9 laboratories using the previously published EuroFlow 5-tube-8-color B-cell chronic lymphoproliferative disease antibody panel. Expression levels of a… Show more

“…FlowSOM largely confirmed the phenotypic heterogeneity found with the manual analysis. The results are in line with other analyses showing that FL are often predicted by different computational approaches to be normal B cells (Böttcher et al, 2022; Shih et al, 2013; Zhao et al, 2020). This underscores the need for more discriminating markers for FL.…”

“…Because the HCL score does not reflect heterogeneity in expression (bright or dim and percentage of abnormal cells that expressed the marker), the use of raw flow cytometry data independent of the cut off could help to classify HCL diseases. Independent classification using immunophenotypic variables has been successfully used in mature B cell neoplasms [ 39 ], but our knowledge has not been tested to discriminate villous lymphoproliferative neoplasms. Unsupervised analysis based on all immunophenotypic markers allowed good discrimination between cHCL and vHCL/SDRPL independent of the immunologic HCL score and BRAF status.…”
