Objective To use scoping review methods to construct a conceptual framework based on current evidence of group wellchild care to guide future practice and research. Methods We conducted a scoping review using Arksey and O'Malley's (2005) six stages. We used constructs from the Consolidated Framework for Implementation Research and the quadruple aim of health care improvement to guide the construction of the conceptual framework. ResultsThe resulting conceptual framework is a synthesis of the key concepts of group well-child care, beginning with a call for a system redesign of well-child care to improve outcomes while acknowledging the theoretical antecedents structuring the rationale that supports the model. Inputs of group well-child care include health systems contexts; administration/ logistics; clinical setting; group care clinic team; community/patient population; and curriculum development and training. The core components of group well-child care included structure (e.g., group size, facilitators), content (e.g., health assessments, service linkages). and process (e.g., interactive learning and community building). We found clinical outcomes in all four dimensions of the quadruple aim of healthcare. Conclusion Our conceptual framework can guide model implementation and identifies several outcomes that can be used to harmonize model evaluation and research. Future research and practice can use the conceptual framework as a tool to standardize model implementation and evaluation and generate evidence to inform future healthcare policy and practice.

Counseling parents about child feeding: a qualitative evaluation of French doctors and health/childcare professionals’ experiences and perception of a brochure containing new recommendations

Background Parents are crucial in establishing their children’s eating habits, and doctors and health/childcare professionals (HCCPs) can provide meaningful and trusted guidance on feeding, especially in the 0-3-year-old period. With the upcoming release of the official brochure containing the new child feeding recommendations in France, this study aims to: (1) assess professionals’ practices and perceptions regarding their communication with parents on child feeding and (2) evaluate their perception of the draft of the new brochure. Methods A 15-page draft brochure (without pictures) containing updated child feeding recommendations for children 0–3 years old was developed by Santé publique France (the French public health agency). Online semi-structured interviews were conducted with professionals (n = 21), including 13 pediatricians and general practitioners (doctors) and eight healthcare or childcare professionals (HCCPs) two weeks after they were provided with this draft brochure to read. The interview guide was developed and piloted with other professionals (n = 3) prior to these interviews. Interview data were transcribed verbatim and analyzed thematically using an inductive approach. Results While doctors and HCCPs mostly communicate orally with parents, both acknowledged that the brochure might be a helpful supplement, especially for HCCPs to legitimize their advice to parents. For doctors, giving the brochure to parents may help provide systematic advice and save time during consultations. Professionals serving parents of lower socioeconomic status would prefer a supplement with less text and more illustrations. In general, the messages were perceived to be easily understandable but providing detachable cards to distribute according to the child’s age would facilitate information dissemination and might be more useful to parents. Professionals reported that lack of training, the circulation of contradictory information, and language barriers were common challenges. Conclusion French professionals welcomed the new official brochure as a means to spread updated child feeding recommendations. However, this brochure could be modified and specific tools developed to better adapt to professionals’ needs of communication with parents and to facilitate the relay of information. Providing updated and consistent information to parents should be considered a priority for public health stakeholders toward increased adherence to new recommendations.

Impact of a Nutritional Intervention Based on Amaranth Flour Consumption to Recovery Undernourished Children

Amaranth flour (AF) can be used as an alternative for nutritional recovery, however, there is not enough evidence on the efficacy of these types of programs. The objective of this study was to assess the effect of an Integral Nutritional Recovery Program (INRP) with AF as complementary feeding on anthropometry in children with grade 1 malnutrition. A quantitative before-after study with intervention and follow up was developed. Dyads of caregiver and grade one malnourished children were evaluated (Control group n=37 and experimental group n=46). A supplement based on amaranth flour were given to Control group (CG) and Experimental Group (EG), only EG received the intervention IPNR. Variables evaluated were weight/height (W/H), muscle mass (MM), fat mass (FM), arm circumference (AC), head circumference (HC), triceps skinfold (TRSF), subscapular skinfold (SSF), body mass index (BMI), subscapular/triceps index (SS/TR), and centripetal index (CI). Data were analyzed using ANOVA, orthogonal arrangements, and paired-T test, 95% CI, significance p≤0.05. Initial anthropometric evaluation showed that the conformation of the CG and EG was homogeneous (p ≥ 0.05). After three months of follow up height and HC showed no differences (p ≥ 0.05). In contrast, W/H, AC, SSF, TRSF, BMI, and FM increased in different magnitudes in function of time for both groups. Paired analysis indicated higher increases in EG than CG. Subscapular/triceps index decreased, and MM showed a tendency to increase (p ≤ 0.1). No significant differences were observed between males and females, height and HC (p ≥ 0.05). Integral Nutritional Recovery Program was effective in improving nutritional status of experimental group and continuing for three months. A follow up is recommended after six months to verify nutritional recovery.
