Extended Lower Limb Venous Ultrasound for the Diagnosis of Proximal and Distal Vein Thrombosis in Asymptomatic Patients after Total Hip Replacement

Abstract: The incidence of asymptomatic DVT is still significant despite prophylaxis but most DVTs remain distal and occur in the first 2 weeks. Extended US could replace phlebography for systematic screening in clinical trials using surrogate endpoints in view of its high accuracy and reliability.

“…First, CTA-CTV was limited to one use on postoperative day 7. A prospective study of 70 patients using repeated ultrasonography revealed that DVT occurs mostly in the first two weeks after THP [8,16]. Thus, the true incidence of asymptomatic VTE may be slightly higher than what we observed in our patients.…”

“…Age and obesity are well-recognized risk factors for VTE after surgery in general [16]. Mantilla et al [7] showed that obesity, poor ASA physical status, and lack of thromboprophylaxis are independent risk factors for symptomatic VTE in their study of 9,791 patients with primary hip or knee arthroplasty.…”

“…However, these procedures have an increased risk of venous thromboembolism with an incidence of up to 84% (1-8). Deep vein thrombosis (DVT) may progress to a potentially fatal pulmonary embolism and also secondary complications such as postthrombotic syndrome, recurrent DVT and chronic pulmonary hypertension (9, 10).…”

“…The diagnostic modalities for detecting the DVT that develops after total joint replacement includes conventional venography (11, 12), Doppler sonography (1-3,13-26), MR venography (27) and radionuclide venography (28). Compared with conventional venography, Doppler sonography has the advantage of being portable, painless and noninvasive, there are no side effects and it is a less expensive screening and surveillance tool for managing DVT (13).…”

“…13 The anterior tibial veins were not investigated in the majority of the patients as anterior tibial veins are rarely affected by DVT. 14 DVT diagnosis was confirmed if there was the presence of endoluminal material combined with lack of compression of the vein. Clot diameter was measured in the transverse plane during maximal compression.…”
