Extensive Extranodal Involvement of Rare Sites in Non Hodgkin’s Lymphoma Detected on 18F- FDG PET-CT: A Case Report

Abstract: We present a case of a 23 year-old male treated for Hodgkin's lymphoma who developed diffuse large Bcell lymphoma (DLBCL) 8 years after achieving remission. 18 F-fluorodeoxyglucose positron emission tomography computed tomography ( 18 F-FDG PET-CT) was done, which revealed extensive extranodal involvement of bilateral atria, bilateral kidneys, ileo-caecal junction and left testis along with mesenteric and retroperitoneal lymph nodal involvement. Renal and cardiac lesions were not detected by contrast-enhanced … Show more

“…[11] Imaging procedures alone cannot differentiate between primary and secondary renal lymphoma as 18 F-FDG is excreted by the kidneys, this physiologic uptake in the renal collecting system can lead to false-positive and false-negative interpretations on PET-CT. [12] Renal involvement in lymphoma is seen as multiple focal areas of increased 18 F-FDG uptake mostly in renal cortices that may be unilateral or less commonly, bilateral. [13] As in our case, where multiple focal areas of increased FDG uptake were noted in both kidneys.…”

“…Based on these attributes, the malignancy may raid any organ system. NHL can affect the gastrointestinal tract, a phenomenon observed in 10% - 30% of all patients with the disease [ 5 ]. However, bowel intussusception in the adult demographic is uncommon and accounts for only 5% of the total tally of intussusceptions observed.…”

“…Whole-body imaging with 18F-FDG PET-CT is obligatory to assess the extent of disease by detecting unexpected extranodal sites of disease or for exclusion of disease in the presence of nonspecific extranodal CT findings [24,25]. In the present case, in addition to the CT findings, involvement of the thyroid gland and mediastinum was determined by 18F-FDG PET-CT.…”
