External Fixation of Proximal Tracheal Airway Stents: A Modified Technique

Abstract: Treatment of subglottic and proximal tracheal stenosis for nonsurgical candidates includes tracheostomy, Montgomery T tubes, and silicone stents. When used in lesions with concomitant malacia, silicone stents have a high incidence of migration. We describe a simple and effective technique of securing endoluminal stents using an Endo Close suturing device (Coviden, Boston, MA) and an external silicone button in 9 consecutive patients.

“…In patients who are unfit for surgery (high-risk patients, patients refusing surgical options), tracheal stents are a good option [16, 17]. Recently, studies have been initiated to investigate treatment options (a tissue-engineered airway, revascularized allografts in the heterotopic position, and cryopreserved aortic allografts) for extensive lesions in postintubation tracheal stenosis [1820].…”

“…This aspect made this antimigration strategy very important, to maintain the stent position covering the fistula. Apart from 1 patient in the series by Majid et al [10] , our series is the only one to describe external fixation of the metal stents as an effective strategy in such a situation.…”

“…In addition, the fixation device is not commonly available. Majid et al [10] also described an external fixation procedure in 9 patients, creating a similar loop, using a special endo-close suturing device with a notched stylet as a suture recovery device to complete the loop. In addition, a silicone button was incorporated on the skin surface as a base to complete the knot.…”

“…Complications of airway stents include: granulation formation, partial/complete airway occlusion, infection, biofilm formation, and stent migration. In particular, stent migration has been commonly reported in the literature, ranging from 16–34% . Techniques to prevent migration include appropriate stent size, design of stents, or methods to secure the stent in place .…”

“…In particular, stent migration has been commonly reported in the literature, ranging from 16–34% . Techniques to prevent migration include appropriate stent size, design of stents, or methods to secure the stent in place . Multiple techniques to endoscopically secure stents or keels exist, but these are oftentimes tedious and cumbersome, and can even require devoted surgical instruments (ie, Lichtenberger needle) .…”
