Extranodal lymphoma originating in the gluteal muscle with adjacent bone involvement and mimicking a soft tissue sarcoma

Abstract: IntroductionExtranodal lymphoma (ENL) in the muscles is a rare manifestation of non-Hodgkin lymphoma (NHL). The aim of this case report is to describe and evaluate the clinical presentation and important radiologic features of ENL affecting the musculoskeletal system.Presentation of caseWe present a 52-year-old female with a 3-week history of left gluteal pain. Computed tomography (CT) showed a non-uniformly early enhancing mass in the left gluteal muscle, the tumor demonstrating central necrosis and adjacent … Show more

“…CT scan may reveal a hypodense or isodense mass, muscle swelling, disruption and injury of the surrounding tissues, periosteal reaction, and sequestration. [ 2 , 3 , 6 ] On MRI, muscle infiltration demonstrates an increased signal intensity on T1-weighted images, intermediate signal intensity compared with fat on T2-weighted images, and homogenous diffuse enhancement when contrast material is used. This specific radiographic evidence assists in the differential diagnosis of muscle lymphoma.…”

“…The soft tissue involvement is more common in NHL rather than Hodgkin's lymphoma [18]. There are several theories put forward suggesting the cause of primary soft tissue lymphomas.…”

“…Patients with extranodal NHL present more frequently with early stage disease compared with those with nodal NHL; overall survival in both groups largely depends on IPI (international prognostic index) and not on the site of origin of the malignancy [ 41 ]. Hence, although cases of primary skeletal DLBCL may be undertreated overall, if diagnosed early and correctly patients have a likelihood to achieve complete response upon treatment with R-CHOP [ 5 , 42 , 43 ].…”
