Background: Efficiency measurement has been of great interest as organizations attempt to improve their efficiency and productivity. Most worked were emphasised on efficiency measurement in hospitals. Focus were on hospitals to established and compared their relative productivity, considering the need to effectively utilize scarce resources available within them. DEA is one of measurement tool commonly used in hospital efficiency study. DEA requires some model specification when use to examine technical efficiency of the hospital. This manuscript aim was to identify the model specification of DEA commonly used in measuring the technical efficiency of hospital. Materials and Methods:Three databases, namely PubMed, CIHAHL and ScienceDirect were used for searching articles from 2013 to 2018. The search technique will involve the use of key words, "hospital", "hospital inputs", "technical efficiency", "hospital efficiency", "hospital outputs", together with the "data envelopment analysis or DEA. Searching and screening were based on PRISAMA procedure.Result: Twenty articles had extracted as a final result of the systematic review process. The number of DMUs was ranged between 9 and 322. Studies were varies in term on DEA model specification, and some studies were similar with other studies in regard to components of DEA model specification. Conclusion:The DEA model specification has an ability to be customized based on researcher preferences and the objective of the study in order to measure hospital technical efficiency.

Jordan is committed to providing healthcare services for more than a million refugees coming from bordering countries in the last five years and face increasing demand for the services of public hospitals. Their efficiency is a key success factor to manage the unique and complex context effectively. This research investigated the technical and scale efficiency of the Jordanian Public Hospitals. The study applied constant and variable returns to scale input-oriented DEA models to rank hospitals and allocate the factors associated with inefficient operations. The work tested 27 general public hospitals from 2010 to 2014, in total, 135 observations were examined with respect to four input-measures and three output-measures. The output-measures characterize three functional areas; inpatient, outpatient, and ambulance and emergency departments. Further, decomposing the technical efficiency allowed considering for scale effects. Findings revealed that 25 observations out of 135 ones were efficient and constructed the efficient frontier. Eight hospitals in 2014 were on the frontier, but weakly efficient and all suffered slacks. Targets and reference sets were identified to guide improvements. Hospitals were sorted into five performance patterns; promising, declining, stable good, stable poor, and unstable. The number of physicians and outpatient services recorded high slacks. On average, 2013 scored the best performance. Scale analysis shows that a capacity of 160 beds is an optimal production size in Jordan. Inefficient and weakly efficient hospitals can target areas of opportunities for performance improvements. The efficiency of Public hospitals in Jordan was not investigated since 1992. The study was limited to public hospitals from 2010 to 2014.

Outsourcing is used as a strategy to improve efficiency, quality, and satisfaction of beneficiaries in hospitals. This study aimed to determine the reasons for outsourcing, outsourced services, achieved objectives, and outsourcing challenges. This qualitative study was conducted through 58 face-to-face semi-structured interviews with chief executive officers (CEOs), chief financial officers (CFOs), and contracting experts (CEs) in 42 public hospitals in Tehran, Iran. Purposive and snowball sampling techniques were used. Interviews were recorded digitally and transcribed verbatim. Data analysis was performed based on an inductive-deductive approach via MAXQDA-10. The Participants included 26 CEOs, 15 CFOs, and 17 CEs. The most important reasons for outsourcing were divided into four main categories including financial resources and funding, customers, management, and factors related to the hospitals. Accessing to up-to-date knowledge and technology, improving service quality and accountability, focusing on strategic points, reducing workload, and increasing patient satisfaction turned out to be the positive outcomes of outsourcing. Weakness in laws and regulations, monitoring and assessment of an outsourced unit, creating dependency and changing organizational culture, as well as human resource issues were introduced as the main pitfalls and challenges of outsourcing. Based on the results, it was suggested that the hospital managers must attempt to outsource services by realistic analysis of the organization's current status. They should specify the reasons and objectives of outsourcing and make decisions based on outsourcing potentials. By taking the current challenges of outsourcing in Iranian hospitals into account, the authorities can make the most efficient decisions with regard to outsourcing.

Abstract: Most of the studied hospitals were operating at low efficiency. Therefore, policymakers should plan to improve the hospital operations and promote hospitals to an optimal level of efficiency.
