Factors associated with inhaled corticosteroids prescription in primary care patients with COPD: A cross-sectional study in the Balearic Islands (Spain)

Abstract: KEY MESSAGESThe proportion of COPD patients with ICS prescriptions in Balearic primary care is lower than proportions observed in other studies. Asthma comorbidity, exacerbations, severe bronchial-obstruction, smoking status and spirometry-registered data were associated with ICS prescription. High-dose ICS prescription was associated with severe obstruction, exacerbations and co-morbid asthma. ABSTRACTBackground: There is a worldwide over-prescription of inhaled corticosteroids (ICS) in patients with chronic … Show more

“…As expected, severe and very severe airflow limitation, comorbidity of asthma, frequent exacerbations, and contact with secondary care were associated with ICS-containing therapy, and this is also supported by other studies 5,30. Among the patients who would benefit from ICS treatment (Table 1), the patients with “possible indication for ICS” had a better lung function when compared with the patients with “appropriate therapy with ICS”, which might be a reason why they did not have ICS treatment despite reported comorbidity of asthma.…”

“…Over two-thirds of COPD patients between 2014 and 2018 were prescribed ICS-containing inhaled therapy, in particular triple therapy; similar to the proportion seen in Sweden but higher than that found in the Balearic Islands (Spain). 24,25 In keeping with guideline changes and recommendations during this time period, the proportion prescribed non-ICS inhaled therapy steadily…”

“…Despite the valuable information these studies provide, they are frequently based on automated databases that do not allow to understand the determinants of pharmacological prescription in daily clinical practice. Although some previous studies have assessed this aspect, 6 , 7 , 9 , 10 most of these studies are conducted in primary care and evaluate only partial aspects of drug prescription. Accordingly, there is a need for a prospective, comprehensive evaluation on what are the key elements on which physicians base their therapeutic decisions in secondary care.…”
