Factors Predicting Long-Term Survival Following Pancreatic Resection for Ductal Adenocarcinoma of the Pancreas: 40 Years of Experience

Abstract: Nearly 20% of patients with pancreas carcinoma who undergo surgical resection have a chance of long-term survival. Survival beyond 5 years is predicted by clinical and tumour-specific factors. Adjuvant chemotherapy might prolong overall survival but is, according to these results, unable to contribute to long-term survival. There is still a risk of recurrence after a 5- or even a 12-year mark. Survival beyond 5 or even 12 years, therefore, does not assure cure.

“…There were significant differences (p= 0.0002). Similar results were observed in previous reports 22,23) . In the present study, nodal metastasis was found to be a marginally significant factor according to a univariate analysis, although it did not reach statistical significance in the multivariate analysis.…”

“…Furthermore, three studies reporting on a similar study population as other eligible studies were included in the systematic review but excluded from the subsequent meta-analysis. [18][19][20] Additionally, one study that analyzed patients using the Surveillance, Epidemiology, and End Results (SEER) national cancer database was excluded from the meta-analysis, as it likely included patients from other eligible studies. 18 Therefore, after the exclusion of ineligible studies, 23 original studies (4339 patients) were included in the systematic review.…”

“…First, the intrinsic nature of a population-based study does not allow reevaluation of histologic specimens. However, other authors 3 have noticed that histologic misclassification is identified in less Abbreviation: NOS, not otherwise specified. a The model-based score was derived from the β coefficient • 10/3 and rounded down to the nearest integer.…”

“…5,6 Long-term survivors (LTSs) represent a particular subgroup of patients with PADC that remains poorly understood. In recent years, several studies 3,[7][8][9] have focused on LTSs (surviving ≥10 years) of PADC to characterize a subgroup of patients for whom definitive cure is possible. These studies are often single-institution reports analyzing a limited number of LTSs, often insufficient to reach definitive conclusions.…”
