“…Our research F I G U R E 1 Elements that influence on the standardised care plan individualisation process confirms the results of previous studies in which nurses say that one of the reasons they do not individualise SCP is because they feel they have an excessive workload (Akhu-Zaheya et al, 2018;Banamwana & Smith, 2015;Pöder, Fogelberg-Dahm, & Wadesten, 2011). In addition is the fact that nurses perceive they have been allocated too many patients, affecting the quality and safety of the care (Aiken et al, 2014;Cho et al, 2015;Kim & Bae, 2018), as well as the use of individualised care plans (Agyeman-Yeboah, Korsah, & Okrah, 2017;Mayilvaganan, 2002;Rose, 2016). There is an increasing amount of evidence that the combination of excessive workloads and nurse-patient ratios negatively affect care individualisation, including its recording (Ballantyne, 2016;Radwin & Alster, 2002;Rose, 2016).…”

“…Individualisation is time-consuming (Agyeman-Yeboah et al, 2017;Chang et al, 2016). If it is also born in mind that some nurses feel that recording reduces the amount of time they spend directly caring for the patient (Rouleau et al, 2017) or, due to their lack of knowledge, individualisation takes them longer, the result is that SCP individualisation process is no longer considered a priority (Ballantyne, 2016;Lima & Melo, 2012).…”

The quality of clinical documentation has therefore been the object of policies, norms and guidelines of health services and of organizations which guide or regulate health actions.…”

“…Agyeman-Yeboah, Ameyah Korsah, and Okrah (2017) determined that the nursing process is not utilized efficiently in the clinical setting in Ghana. Factors influencing this include a lack of understanding by the nurses of the nursing process as a tool, a lack of access to nursing care plans, and limited time to have the opportunity to implement a care plan (Agyeman-Yeboah, Ameyah Korsah, & Okrah, 2017).…”
